• Yoga Nidra Intake Form

    Please fill out as much of this form as you are comfortable sharing. This is NOT a test! Try not to overthink any of the questions and simply respond with whatever comes to mind first. You are not required to fill out every question, but the more you share, the more refined your Yoga Nidra experience can be. There are no right or wrong answers! All information shared will remain 100% confidential between you and your Nidra guide, Dana Ripley of Embodied Mind Healing. All recordings are custom made to address your present moment needs and interests, with the overarching intention to bring the listener into a deep state of rest and relaxation. All in-person sessions may include sound healing, aromatherapy, movement, or energy work upon request.

    We recomend tuning in at least once a week, and encourage you to try a daily Yoga Nidra practice as well. If time isn't on your side, simply listen as you fall asleep at night or set your alarm early in the morning to check in with yourself before the start of your day.
