Media Release and Attendance Agreement
I, parent or official guardian of (child’s name listed above) hereby grant permission to the Washington Urban Debate League (WUDL), The Matthew Harris Ornstein Memorial Foundation, and/or persons contracted by WUDL, to take photographs and/or video/audio recordings of my child in connection with his/her participation in debate programs. Furthermore, I grant permission to the same entities to publish in print, electronic, or video format the likeness or image of my child obtained during debate competitions for public relations, educational, documentary film on Urban Debate or any other lawful purposes. I further agree that my child’s name and identity may be revealed in connection with the likeness or image. I authorize the use of my child’s likeness or image without compensation to my child or me. All negatives, positives, prints, digital reproductions, and video recordings shall be the property of whichever entity they are taken by: WUDL, Matthew Harris Ornstein Memorial Foundation. I release all claims against the same entities with respect to copyright ownership and publication including any claim for compensation related to use of the materials.*
I (parent/guardian) agree that my child(ren) will attend the summer institute. We assume that those who complete this application will attend the institute, and make substantial resource allocation decisions based on sign ups. If your ability to attend changes, we expect families to notify us as soon as possible. Failure to show without cause will result in being unable to register for future summer programs as doing so impacts our ability to keep the institute free and accessible to all. *
