Please select the days & times for your sessions below. You will only have to fill this form out once. This will be your permanent days and time (unless you request to change) You may request a certain time if needed.

The prices below are is a one time monthly payment

Session Recommended

• Beginner- 4-5 sessions
• Intermediate- 3-4 sessions
• Advance- 3 sessions
• Weight lost - 4-5 sessions
• Weight gain - 4 sessions
• Tone body- 3 sessions 
• Stay in shape- 2 sessions

•Sessions last for 1hr.

•ALL CLIENTS MUST HAVE A PLANET FITNESS MEMBERSHIP. 

•Monthly Payments Only 

•You MUST make a $50 deposit  when booking. (Will show up at the end of the form) 

•After deposit is sent, you will receive a text with the remaining balance of your total

•Your remaining balance MUST BE PAID by 5:30pm the day before your first session. Payments made after 5:30pm must pay a $15 late fee. Failure to make payment will equal no call no show 

• Only 1 reschedule is allowed from start date

• Must schedule your start date within 2 weeks from submission date.

• If client does not call or a no show, and will like to sign back up, client must pay a $35 restart fee, in order to start first workout session. 

• ALL PAYMENTS ARE NON-REFUNDABLE

 

 

If client have any medical/health conditions the client must get a physician’s clearance in order to participate.
