Health Information Release. This health history is correct and complete, as far as I know. I hereby authorize Denton Camp's health officials to share health information and health history with the other staff members on a need to know basis. This includes the camp director, program directors, and any other necessary staff. The purpose of this disclosure is for the necessary staff to be prepared in advance for any medical emergencies. I agree to the release of any records necessary for insurance purposes.
Basic First Aid & Medication Authorization. I hereby give permission to the camp to provide basic first aid and seek any needed medical care.
For minors, I give permission to camp staff to administer prescribed medications as authorized by my child's PCP. I also give permission for Denton Camp to administer camp stocked over-the-counter medications on an "as needed" basis, as indicated on the health form, and as directed by the health supervisor. I give permission to Denton Camp to seek emergency medical treatment including ordering x-rays or routine tests. In the event of an emergency, I give permission to the camp to arrange necessary related transportation for my child. In the event I cannot be reached in an emergency, I hereby give permission to the practitioner selected by the camp to secure and administer treatment, including hospitalization, for my child.
Medication Policy. Campers and camp staff are not permitted to keep medication in luggage or dorms (exception: emergency medications). All adult staff medication will be self-administered and stored securely in the nurse’s station, private cabin, or personal vehicle and not stored in dorms where minors are housed.
For staff under 18 and campers, all medications (including prescription, over-the-counter medications, vitamins, and supplements) brought to camp MUST be turned in to health staff at check-in. All medication must be sent in the unaltered original manufacturer’s package or pharmacy labeled bottles. Do Not use Daily Pill Boxes. Send only the amount of medication needed for while at camp. The pharmacy labeled bottles must match the practitioner’s written order on the Medication Administration Authorization form. All medication for campers and staff under 18 will be administered by camp nurses who will document all medication administered. All medication must be retrieved at check-out. Medication left behind will be discarded.
Indemnity Release. I agree that Denton Camp, its agents, officers, employees, trustees and volunteers will not be liable for any injury, death, damage and/or loss to myself or my child, and/or anyone claiming on my or my child's behalf, and I further agree to hold harmless, indemnify and defend Denton Camp, its officers, staff, agents, employees, trustees and volunteers for and from any and all liability, claims, losses, injuries, expenses, fees and/or damages arising out of any injury, illness or death to myself or my child or property damage during my or my child's attendance at Denton Camp. The minor child herein has permission to engage in all camp activities.
Media Release. I grant permission to Denton Camp, its agents, and its employees the irrevocable and unrestricted right to produce photographs and video taken of my child, myself, and members of my family while at Denton Camp for any lawful purpose including publication, promotion, illustration, advertising, trade, or historical archive in any manner or in any medium. I hereby release Denton Camp and its legal representatives from liability for any violation or claims relating to said images or video. Furthermore, I grant permission to use the statements of my child, myself, or my family members given during an interview or evaluation with or without my name for the purpose of advertising and publicity without restriction to time limit or geographic area. I waive my right, my child’s rights, and my family’s rights to any and all compensation stemming from the use of these materials.
Participation Agreement. In consideration for the opportunity to participate in camp activities, the Participant (or parent/guardian if the participant is a minor) acknowledges and accepts the risk of injury associated with participation in and transportation to and from the activity. The Participant (or parent/guardian accepts personal financial responsibility for any injury sustained during the activity or during transportation to and from the activity. Further, the Participant (or parent/guardian) promises to indemnify, defend and hold harmless the activity sponsor or its agents, employees, volunteers, or any other representatives (collectively referred to hereinafter as the “Sponsor”) for any injury related directly or indirectly out of the described activity or transportation to and from the activity, whether such injury arises out of the negligence of the Sponsor or otherwise. If a dispute over this agreement or any claim of damages arises, the Participant (or parent/guardian) agrees to resolve the matter through a mutually acceptable alternative dispute resolution process. If the Participant (or parent/guardian) and the Sponsor cannot agree upon such a process, the dispute will be submitted to a three-member arbitration panel of the American Arbitration Association for final resolution. Description of Summer Camp Activities: Activities include, but are not limited to: canoeing, fishing, swimming and water activities, group games, team building activities, and off-site activities.
