Thank you for choosing Design Wave 4. Please be advised that this form should ONLY be completed once you have completed your payment or deposit. (Services over $200 require a 50% deposit before starting on selected service). ALL other services must be PAID IN FULL before booking.
TURN AROUND TIME
All turn around times start once you have submitted all information needed for your service. The turn around time is 7-14 business days, NOT including weekends or holidays. If you need a rush order (1-2 business days) turn around, There is an ADDITIONAL FEE of $50 THAT MUST BE PAID AT BOOKING.
