1. Recipient is available at least 18 years of age.

2. This is a one on one session.

3. Everything discussed is confidential and will not be recorded.

4. Meetings are 1 hour long but additional meetings can be scheduled. 

 

Dignify a Diva, Inc is a 501c3 nonprofit organization. It understood that all liability will be released from Dignify a Diva. 

It is the responsibility of the Sparkle & Wellness recipient to ensure all health needs. If there are no mentions of health issues we will operate the program with an understanding that the recipient is healthy and able to participate. 

We do not claim to fix situations nor offer permanent fixes to situations. We aren't able to promise help in the form that is expected by others. We are only able to give what's within our capacity to offer. 

We do not claim nor guarantee that the resources we give upon request will birth a happy ending. We only give the resources that are available within the state of Georgia, we do not have control over their application process. 

It is understood that all liability will be released from Dignify a Diva in the event of a crisis situation. We will do our best to assist in the safest way possible.
