    Thank you for booking your experience with Two Oceans Aquarium Dive School.Please complete the booking form below to reserve your place and help us ensure that everything is prepared for you on the day, with no delays or disappointment
  • Please note that each participant must complete their own booking form using their own personal details. The information provided on this form is used to confirm the participant’s booking, ensure we have the correct contact details, and help our team prepare for the dive safely and efficiently.

    Each participant must have their own email address, as all booking confirmations, important dive information, indemnity details, and any updates regarding weather, dive conditions, meeting times, or changes to the schedule will be sent directly to that email address. Multiple participants cannot be booked under one person’s details or email address, as this may cause delays with communication, check-in, and dive administration.

    Before completing this booking form, please note that shore and boat dives are weather and condition dependent. Dive sites, meeting times, and final arrangements may be adjusted closer to the time to ensure the safest and best possible diving experience. Boat dives are also subject to sea conditions, skipper approval, and minimum numbers where applicable. Your booking will only be confirmed once the Dive School has checked availability and confirmed your selected dive date

  • Please select your preferred shore or boat dive date on the calendar. All shore and boat dives are weather and condition dependent, and your selected date is not automatically confirmed.   • As part of this booking form, you will be required to complete the medical questionnaire. Please note that this medical questionnaire will also need to be completed again once your PADI eLearning materials have been assigned, as PADI requires this information to be verified through their system as well. The questionnaire on this form helps us check, before confirming your booking, whether you may need medical clearance from a physician before taking part in any scuba diving activities.

    Please read each medical question carefully and answer honestly. Certain medical answers may require a physician’s approval before you can continue with the course. If this is only identified after your eLearning has already been assigned, it may result in your course being postponed or cancelled, and no refund may be possible for as eLearning materials that have already been issued.

  • (1) I have had problems with my lungs, breathing, heart and/or blood affecting my normal physical or mental performance.*

  • (4) I have had problems with my eyes, ears, or nasal passages/sinuses.*

  • FOR SHORE AND BOAT SCUBA DIVES
    I, the undersigned, acknowledge that scuba diving involves inherent risks and hazards, including but not limited to changing sea and weather conditions, currents, surge, waves, reduced visibility, marine life, boat operations, entry and exit points, and the general risks associated with scuba diving in the ocean.

    I am fully aware that these risks may result in personal injury, loss, damage, serious injury, or death.

    I confirm that I am in good health and have disclosed any relevant medical conditions, concerns, or recent changes in my health to the dive team before participating in the dive.

    I confirm that I am a certified scuba diver, or a student diver participating under the control and supervision of a certified scuba instructor, where applicable.

    I hereby indemnify and hold harmless the Two Oceans Aquarium Trust (“the Aquarium”), its trustees, employees, independent contractors, agents, representatives, dive professionals, boat crew, skipper, and any person acting on behalf of the Aquarium in connection with the dive, against any claim or claims by myself or by any other person, persons, or body whatsoever, for any injury, damage, loss, or death which may be sustained by me or any other person arising from my participation in the shore or boat scuba dive.

    I further agree to follow all rules, regulations, safety briefings, and instructions provided by the Two Oceans Aquarium Dive School, dive professionals, boat crew, and skipper before, during, and after the dive.

    I understand that all shore and boat dives are weather and condition dependent, and that the dive site, dive time, dive date, or dive plan may be changed, postponed, or cancelled by the Dive School, dive professional, boat crew, or skipper for safety reasons.

    I understand and agree that PADI Members, including Two Oceans Aquarium Dive School and any PADI Instructors or Divemasters associated with the programme or dive activity, are licensed to conduct PADI training and activities but are not agents, employees, or franchisees of PADI Americas, Inc., or its parent, subsidiary, or affiliated corporations.

    I further understand that Two Oceans Aquarium Dive School operates independently and that PADI does not own, operate, or control the day-to-day business activities, supervision, or dive operations of the Dive School or its associated staff.

    I understand and agree that neither PADI nor its affiliated companies may be held liable for the actions, inactions, or negligence of Two Oceans Aquarium Dive School, its instructors, divemasters, staff, contractors, or representatives.

    I understand that I am voluntarily participating in recreational scuba diving activities, which may include shore diving, boat diving, snorkelling, boating, and related ocean-based activities. I certify that I am fully aware of and expressly assume all risks involved in these activities.

    I understand and agree that I am giving up certain legal rights by signing this agreement, including the right to bring a claim against the released parties for injury, loss, damage, or death arising from my participation, including where such loss may arise from negligence, whether passive or active.

    I confirm that I am of lawful age and legally competent to sign this agreement, or that I have obtained the written consent of my parent or legal guardian where required.

    I have read and understood this indemnity, non-agency disclosure, liability release, and assumption of risk agreement, and I voluntarily and willingly sign it to participate in the shore or boat scuba diving experience.


    I, BY THIS INSTRUMENT, AGREE TO EXEMPT AND RELEASE ALL THE ABOVE LISTED ENTITIES AND/OR INDIVIDUALS, WHETHER SPECIFICALLY NAMED OR NOT, FROM ALL LIABILITY AND RESPONSIBILITY FOR PERSONAL INJURY, PROPERTY DAMAGE OR WRONGFUL DEATH, HOWEVER CAUSED, INCLUDING, BUT NOT LIMITED TO, PRODUCT LIABILITY OR THE NEGLIGENCE OF THE RELEASED PARTIES, WHETHER PASSIVE OR ACTIVE.
    I HAVE FULLY INFORMED MYSELF AND MY HEIRS OF THE CONTENTS OF THIS NON-AGENCY DISCLOSURE AND ACKNOWLEDGEMENT AGREEMENT AND LIABILITY RELEASE AND ASSUMPTION OF RISK AGREEMENT BY READING BOTH BEFORE SIGNING BELOW ON BEHALF OF MYSELF AND MY HEIRS.

     

  • Terms and conditions – cancellation and/or postponement

    • It must be completed within 6 months of commencement. We recommend consecutive days but will always do our best to accommodate your needs as best we can.

    • It is essential that all dive medicals are finalized before your scheduled course. Should you have any uncertainties regarding the medical form, please do not hesitate to reach out to us in a timely manner.

    • No Refund Policy: Once e-learning has been assigned to the student, there will be no refunds. This includes situations where a student decides to withdraw or cancel after accessing the e-learning materials.

    • Rescheduling Policy: If you are unable to attend the course on the scheduled dates, you must provide us with 48 hours' notice to reschedule. Failure to give notice or to attend on the scheduled dates may result in forfeiture of your course fees.

    • Weather Permitted: All diving activities are weather dependent. If the weather conditions are unfavourable for diving, the course may be rescheduled

    • If weather conditions or other unforeseen circumstances prevent the dive from taking place, we will reschedule your experience or provide a full refund.Due to the nature of our operations, which are influenced by factors beyond our control
    (including but not limited to weather conditions, safety considerations, and unforeseen circumstances), we reserve the right to amend or reschedule booking dates when necessary. While every effort will be made to adhere to the original schedule, clients acknowledge and accept that changes may be required, and we will communicate and arrange suitable alternative dates as promptly as possible.

  • Your Privacy

  • • Due to the nature of recreational scuba diving and aquarium diving experiences, we may need to collect personal information about you, including information relating to your health and medical fitness to participate safely.

    • We use Jotform to collect the required booking and participant information before your course or experience. This helps us make sure we have the correct details before your activity and reduces the need for multiple back-and-forth emails.

    • Your privacy is very important to us. Unless we have your consent, we will not disclose your personally identifiable information or medical information to third parties. This information will only be used for the purposes directly related to your booking, course, experience, safety, and administration.

    Please note that once your booking form has been submitted, the Dive School will review your request and revert back with the final confirmed course dates, as well as an invoice or payment link. Your booking will only be confirmed once payment has been received. If payment is made on-site, it must be completed before the commencement of the course.
