    Thank you for booking your PADI course with Two Oceans Aquarium Dive School. Please complete the booking form below to reserve your place on one of our upcoming course dates
  • Please note that each participant must complete their own booking form using their own personal details. The information provided on this form is used to assign the correct PADI eLearning materials and to create or link the participant’s individual PADI account.

    Each participant must also have their own email address, as the PADI eLearning access and course information will be linked directly to that email account. Multiple participants cannot be booked under one person’s details or email address, as this may cause delays with eLearning access, certification processing, and course administration.

    Before completing this booking form, please note that all courses run over multiple days and generally take place from 08:00 to 16:00, depending on the course schedule, weather, and dive conditions. Course spaces are limited, with a maximum of 6 students per course slot, and your booking will only be confirmed once the Dive School has checked availability and confirmed your selected dates. Some training days, especially open-water or boat-dive days, are weather dependent and may need to be adjusted closer to the time for safety and best diving conditions.

Specialties are a great way to build extra confidence, improve your diving skills, and get more value from your training.For example, Open Water students may be able to add specialties such as Peak Performance Buoyancy, Underwater Navigation, Boat Diver, or Enriched Air Nitrox.   • Medical Declaration

  • As part of this booking form, you will be required to complete the medical questionnaire. Please note that this medical questionnaire will also need to be completed again once your PADI eLearning materials have been assigned, as PADI requires this information to be verified through their system as well. The questionnaire on this form helps us check, before confirming your booking, whether you may need medical clearance from a physician before taking part in any scuba diving activities.

    Please read each medical question carefully and answer honestly. Certain medical answers may require a physician’s approval before you can continue with the course. If this is only identified after your eLearning has already been assigned, it may result in your course being postponed or cancelled, and no refund may be possible for as eLearning materials that have already been issued.

  • (1) I have had problems with my lungs, breathing, heart and/or blood affecting my normal physical or mental performance.*

  • (3) I have had a close blood relative die suddenly or of cardiac disease or stroke before the age of 50, OR have a family history of heart disease before age 50 (including abnormal heart rhythms, coronary artery disease or cardiomyopathy).*
  • (4) I have had problems with my eyes, ears, or nasal passages/sinuses.*

  • (5) I have had surgery within the last 12 months, OR I have ongoing problems related to past surgery.*
  • (6) I have lost consciousness, had migraine headaches, seizures, stroke, significant head injury, or suffer from persistent neurologic injury or disease.*

  • (7) I am currently undergoing treatment (or have required treatment within the last five years) for psychological problems, personality disorder, panic attacks, or an addiction to drugs or alcohol; or, I have been diagnosed with a learning or developmental disability.*

  • FOR SCUBA DIVING IN THE SHARK, KELP FOREST, & I&J OCEAN EXHIBITS

    I undersigned, acknowledge that scuba diving involves inherent risks and hazards, including but not limited to those posed by underwater predators, the complexity of kelp forest navigation, and the unique challenges of the predator and I & J Ocean exhibit.

    I am fully aware that these risks may result in personal injury, loss, or damage.

    I affirm that I am in good health and have disclosed any relevant medical conditions or concerns to the dive team.

    I hereby indemnify and hold harmless, the Two Oceans Aquarium Trust (“the Aquarium”) its trustees and/ or employees and/or independent contractors and/or agents of the said Aquarium and/or any person representing the Aquarium in connection with such dive against any claim or claims by myself and by any other person, persons or body whatsoever or whomsoever, for any injury or any damage or any loss which may be sustained by me or any person arising from the dive.

    I further agree to abide by all the rules, regulations, and instructions provided by the Two Oceans Aquarium Dive School before, during and after the dive.

    I have read and understood this indemnity form, and I voluntarily and willingly sign it to participate in the scuba diving experience.

     

  • Terms and conditions – cancellation and/or postponement

  •  

    • It must be completed within 6 months of commencement. We recommend consecutive days but will always do our best to accommodate your needs as best we can.

    • It is essential that all dive medicals are finalized before your scheduled course. Should you have any uncertainties regarding the medical form, please do not hesitate to reach out to us in a timely manner.

    • No Refund Policy: Once e-learning has been assigned to the student, there will be no refunds. This includes situations where a student decides to withdraw or cancel after accessing the e-learning materials.

    • Rescheduling Policy: If you are unable to attend the course on the scheduled dates, you must provide us with 48 hours' notice to reschedule. Failure to give notice or to attend on the scheduled dates may result in forfeiture of your course fees.

    • Weather Permitted: All diving activities are weather dependent. If the weather conditions are unfavourable for diving, the course may be rescheduled

    • If weather conditions or other unforeseen circumstances prevent the dive from taking place, we will reschedule your experience or provide a full refund.Due to the nature of our operations, which are influenced by factors beyond our control
    (including but not limited to weather conditions, safety considerations, and unforeseen circumstances), we reserve the right to amend or reschedule booking dates when necessary. While every effort will be made to adhere to the original schedule, clients acknowledge and accept that changes may be required, and we will communicate and arrange suitable alternative dates as promptly as possible.

  • Your Privacy

  • • Due to the nature of recreational scuba diving and aquarium diving experiences, we may need to collect personal information about you, including information relating to your health and medical fitness to participate safely.

    • We use Jotform to collect the required booking and participant information before your course or experience. This helps us make sure we have the correct details before your activity and reduces the need for multiple back-and-forth emails.

    • Your privacy is very important to us. Unless we have your consent, we will not disclose your personally identifiable information or medical information to third parties. This information will only be used for the purposes directly related to your booking, course, experience, safety, and administration.
