  • Terms of Application:

    ENTRANCE REQUIREMENTS: All dogs must be 4 months of age or older. They must be spayed or neutered if over 6 months of age. They must have current Bordetella (Kennel Cough), Distemper and Rabies Vaccines. The Canine Influenza Vaccine is recommended but not required. We recommend that the Bordetella and Canine Influenza Vaccines be administered at least 2 weeks prior to day camp/boarding. Pets must be in good health and flea/tick free. If found to have fleas/ticks, preventative treatment will be given at pet owners expense. Must be non-aggressive. All dogs must successfully complete the Camper Temperament Test. LATE FEES – For boarding, pick-up time at end of reservation is 12:00 PM. If you do not pick up your pet by that time, you will be charged the standard 1/2 day of day camp fee. Your pet(s) will be overnight boarded if you are more than 15 minutes late after closing. You will be charged the standard boarding fee. CANCELLATION POLICY - We ask for you to cancel 48 hours in advance of your day camp or overnight reservation. Day Campers are charged the normal rate for a no-show, and boarders are charged a minimum of a two-night cancellation fee. 72 hours' notice is required to cancel a holiday boarding reservation without a financial penalty. AGGRESSIVE DOGS - We do not accept aggressive dogs into our day camp or boarding programs. Our thorough interview process screens for aggression. If an accepted dog exhibits unacceptable behavior, the dog is given a timeout, and the owner is informed of the incident. We will then work with the canine camper’s owner to resolve the situation; we may conclude at any time that a dog is not appropriate for participation in our day camp and/or boarding programs. FEEDING - It is highly recommended that you bring your dog's own food. If you prefer that The Dog Cabin provide the food, there will be a $2.50 per meal charge. MEDICATIONS - Should your pet require oral medications during their stay; we will be happy to accommodate that. Oral medications will be administered per your instructions at no extra charge. We cannot administer injectable medications. Ear or Eye medications can be administered for a $2.00 fee per administration. You must bring medications clearly labeled with your pet’s name and instructions on dose, frequency, and route (oral, eye, ear). WHAT TO BRING FOR BOARDING - Your dog must have a Quick Release Collar and tag with your current contact information when entering The Dog Cabin. Feel free to bring any toys you'd like them to have overnight. There is no need to bring any bedding as we provide cots and/or blankets. Please be advised that The Dog Cabin cannot be held liable for lost or damaged belongings. Please let us know if your dog is prone to chewing bedding. The Dog Cabin provides bedding to each pet; if destroyed by pet, their owner will be held responsible for replacement cost of up to $30.00. Let us know if you do not want us to provide bedding. PAYMENT POLICY - Fees for Services & Products: You agree to pay for camp fees, services or products with the credit cards listed on the front of this application or by cash or check. By listing your credit card information on the front of this application you are allowing The Dog Cabin, LLC to debit your credit card account for services, fees, or products. You agree to pay an administrative fee for any returned check or debit problems, such as insufficient funds, closed account, frozen or declined credit, or similar circumstances. Refunds: There will be no refunds on services already provided such as daycare, boarding, and grooming. MEMO OF UNDERSTANDING: *I understand that The Dog Cabin, LLC, its owners and employees, will exercise due care to protect the health and safety of my pet while in their care. I understand that The Dog Cabin’s Day Camp and Boarding service is an open play environment and because of this there are inherent risks, which even when monitored, may result in but not limited to the following: a) Transfer of a communicable illness such as canine cough (kennel cough), canine influenza, puppy warts, and parasites. b) Injuries, usually benign, such as broken nails, sore pads, puncture wounds, abrasions, and cuts. If injury, illness, or death occurs, The Dog Cabin will attempt notify me immediately to receive specific instructions on how to proceed or to advise me that my pet(s) is being taken to its veterinarian on file. If The Dog Cabin staff is unable to reach me or my emergency contact, I give permission to The Dog Cabin, LLC to seek any medical attention necessary and agree to pay for all charges incurred. I hereby waive and release The Dog Cabin, LLC, its owners or employees from any and all liability of any nature for any injury, death, or loss of my pet while receiving veterinary care during its stay at The Dog Cabin, LLC. I consent to the release of medical information about my pet to my veterinarian on file. I agree to pay all veterinarian charges incurred. *If I wish to board two or more dogs in one kennel, I understand that I assume liability for any injuries they cause to each other. *By entering my pet into The Dog Cabin, I hereby waive and release The Dog Cabin, LLC, it’s owners or employees from any and all liability of any nature for any injury, death, or loss of my pet resulting from their actions or from the action of my dog or any other dog while in the custody of, on the grounds of, or in the surrounding area of The Dog Cabin. *In the event my dog causes injury to another dog or to a person while at The Dog Cabin, I agree to indemnify and subrogate The Dog Cabin, LLC or it's owners from any action, which may be brought against them, and for any defense, settlement, or judgment against them. I will assume all liability for the actions of my dog and agree to maintain personal liability insurance to cover me in the event of such an incident. *I understand that I may not neglect to pick up my pet by the time they are scheduled for pick up without prior notification and approval by The Dog Cabin, LLC. I further understand that any pet that is left at The Dog Cabin without any notification by me or my representative will be considered abandoned on the 14th day after scheduled pick up. I understand that if I abandon my pet, The Dog Cabin, LLC, by default, will assume primary ownership of my pet and may therefore take any action necessary to re-home them. I understand and agree that if I abandon my pet at the Dog Cabin, LLC I relinquish all my rights to custody of my pet and will have no recourse of action against The Dog Cabin, LLC. I lastly agree that I will still be held responsible for any and all charges incurred while my pet is in the care of The Dog Cabin, LLC including, but not limited to, day camp, overnight boarding, veterinary care, and re-homing expenses.
