    Reports to: Executive Director/ Supervisor
  • Position Description

    Provides personal care services according to care plan, The Direct Care Worker is assigned to specific consumers and performs services for consumers, as necessary, according to their care plan to maintain their personal comfort.
  • Position Qualifications

    1. Successful completion of a written skills test and competency evaluation.
    2. Be at least eighteen (18) years of age.
    3. Demonstrated ability to read, write, and follow a written care plan.
    4. Good verbal, written, and interpersonal skills.
  • Essential Job Functions/ Responsibilities
    1. Performs simple procedures according to the service plan.

    • Range of motion exercises
    • Assistance in ambulation, exercises, or transfer

    2. Performs personal care activities, including but not limited to:

    • Shampooing/hair care

    6. Promotes personal safety and a safe environment for clients by observing infection control practices, following agency guidelines, and reporting unsafe situations to the Executive Director or designee.
    7. Demonstrates safe practice in the use of equipment. Do not use equipment until orientation about its operation has been provided. Notifies supervisor of educational needs.

    8. Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.

    The provision of Discover Our World of Healthcare programs in a quality manner requires a
    corporate commitment to conducting business affairs and consumer care with attention to business ethics and full compliance with applicable federal, state, and local laws, and related payer regulations.

    These standards of business are evidenced in this Code of Conduct for Discover Our World of Healthcare and will be maintained through the ongoing commitment to integrity and ethical standards by Discover Our World of Healthcare officers, employees, and volunteers as they conduct business and consumer care activities.

    Each employee shares personal responsibility for conforming his or her behavior to the expected Code of Conduct. If at any time a question arises as to whether a procedure or action is within the Code of Conduct, the employee should communicate with his/her supervisor for guidance, other resources for guidance include the employee handbook, senior staff, or the Executive Director. The Executive Director is available for all staff to consult confidentiality about any compliance questions or concerns.
     
    Each employee is responsible for reporting possible violations. Alleged violations can be reported by talking with the Executive Director or by drafting a letter (anonymous or signed) directed to the Executive Director. All such communication will be handled on a confidential basis and the reporting party may initially remain anonymous if so desired. The Executive Director will review/investigate all reported concerns and will follow through on whatever action is deemed necessary.
     

    The Code of Conduct is expected to be followed by all Discover Our World of Healthcare employees who provide services to this agency's consumers. Any violation of this Code of Conduct is a serious matter. Employees who violate the Code of Conduct are subject to disciplinary action, up to and including termination,

     
    The Code of Conduct is not inclusive of all organizational policies and procedures but is intended to serve as a guide for behavior related to the organization's highest risk areas related to expected compliance matters. All employees and volunteers/interns are expected to review further policies and procedures guiding them in business practices so that Discover Our World of Healthcare adheres to the highest standards of business and consumer care practices established by the management of this organization.
     
    The Code of Conduct guides employees in adhering to policies and procedures related to Consumer Care, Applicable Laws and Regulations governing Home Care, Business Conduct and Billing practices, and Conflict of Interest matters.

    Discover Our World of Healthcare is committed to providing a high level of quality care for persons needing home care. The safety and well-being of our consumers is of primary importance and we highly respect each consumer's and family's dignity and desire or quality of life arid quality of care.


    Attention to required state, federal, and regulatory bodies' expectations is a part of the provision of consumer care. Discover Our World of Healthcare employees are expected to give such matters their attention throughout consumer care and including personnel licensing requirements, billing documentation and quality improvement matters.
     

    Discover Our World of Healthcare provides care to all consumers needing assistance in a quality manner no matter what the payment source or level of reimbursement.

     
    Compliance with Laws and Regulations
     

    Quality consumer care is a primary goal of all Discover Our World of Healthcare's staff. Such care will be provided in accordance with all state and federal regulations.


    Care of consumers must be medically necessary as required in designated programs. Referrals are received without any one person or any entity being offered free services or special consideration. Consumers may not be offered any financial inducement, gifts, or bribes to encourage their acceptance of treatment from Discover Our World of Healthcare

     
    Providers of care will be in compliance with federal and state requirements governing their

    respective disciplines and Discover Our World of Healthcare's personnel policies will indicate the expected requirements of these disciplines to practice with Discover Our World of Healthcare.

     

    Billing and consumer records must be detailed, accurate, and complete as required by government and regulatory standards.

     

    Billing practices of the organization will adhere to the fiscal policies of the organization. The provision of charity or reduced fee services will be guided by approved corporate procedures and policies.


    Consumers and staff are made aware of state and federal hotlines, the hours of service, and their purposes, which are to receive complaints or questions about home care agencies.


    It is the responsibility of each employee of Discover Our World of Healthcare to assure their understanding of policies and procedures established for the provision of care within expected regulations. Employees are expected to access organizational mechanisms such as policy and procedure manuals, communication with management, and with the Executive Director to improve their understanding so that compliance with all required laws and standards is adhered to at all times.


    Business Conduct
     

    Discover Our World of Healthcare staff involved in daily business practices must maintain high levels of integrity and ethics as billing and related business records are developed.

     
    It is expected that all of Discover Our World of Healthcare's employees and contractors maintain all such records in a complete, accurate, and honest manner. Policies and procedures related to record retention and confidentiality must be followed and no records destroyed unless allowed for in assigned policies and procedures.

    Billing practices are monitored on a regularly established basis. All billing staff is educated and remain informed regarding Discover Our World of Healthcare policies and procedures related to billing so that visits are billed accurately. Billing is to occur only for care pursuant to a plan of care within specified programs and subsequent monitoring is done to assure compliance with this expectation and billing practice.

    Cost reports and related financial documentation are prepared consistent with state and federal requirements.


    Conflict of interest

    Employees and volunteers ofDiscowr Our World of Healthcare are expected to follow the organization's Conflict of interest policy and other guidelines as outlined within the employee handbook, such as those related to non-acceptance of gifts and gratuities while providing services of Discover Our World of Healthcare Decisions made by Discover Our World of Healthcare employees and volunteers should be in the best interest of the organization and its consumers.
     

    The Conflict of Interest Policy states that founding members, staff members, or members of a committee with Board delegated powers have a responsibility to disclose any existing or potential conflict of interest which would affect business decisions or practices affecting Dim Our World of Healthcare.

     
    Each employee, founding member, and volunteer of Discover Our World of Healthcare must recognize his or her own responsibility and accountability for behaviors related to this Code of Conduct and the corporate compliance plan. It is expected that each of these Discover Our World of Healthcare business associates will be evidence of Discover Our World of Healthcare quality of care and quality of business practices through their professorial conduct as a Discover Our World of Healthcare representative.

  • Standards of Conduct Policy

    Policy: Personnel will adhere to Discover Our World of Healthcare standards of conduct in interactions with co-workers and consumers. These standards will apply to all employees including direct care workers and administrators.
     
    Procedure:

    1. Personnel is expected to complete work assignments on time. If an emergency arises, and the employee is unable to complete the assignment he/she must notify the supervisor immediately.
    2. Paperwork will be accurately completed on time. Intentional falsification of documentation in consumer records, billing records, or employment records will result in disciplinary action, including immediate employment termination.
    3. Employees are not permitted to request or accept consumer loans or gifts of material value.
    4. No employee as a result of his/her affiliation with Disccher Our World of Healthcare may assume power of attorney or guardianship over a consumer utilizing the services of this company. Discover Our World of Healthcare may not require a consumer to endorse checks over to the agency.
    5. Employees must follow agency policies, especially those relating to admitting, transferring, and terminating consumers. Billing personnel will follow financial policies to ensure the accuracy of bills and billing practices.
    6. Failure to follow instructions given by a supervisor could result in immediate termination,
    7. Failure to adhere to any of the following could also result in termination:
    • Deliberate or negligent misuse of agency or consumer property.
    • Smoking
    • Use of obscene language
    • Solicitation
    • Excessive absenteeism or tardiness
    • Divulging confidential information concerning consumers, agency personnel, or agency business,
    • Endangering the welfare of others
    • Failure to maintain a professional personal appearance
    • Fighting or causing a disturbance on agency premise of consumer home.

     
    Direct Care Workers Only

    Personnel who deliver services directly to consumers are to follow these guidelines:

    1. Dress according to agency policy
    2. Wear ID badge at all times
    3. Always act in a professional, respectful manner to enhance a spirit of cooperation, mutual respect, and trust with the consumer and other members of the homecare team.
    4. Refrain from inappropriate behavior, including but not limited to impulsive, disruptive, sexually harassing, or disrespectful behavior or derogatory or inflammatory documentation.
    5. Avoid engaging in personal discussion
    6. Avoid voicing personal opinions about consumers and others
    7. Be punctual and responsible
    8. Do not accept cash or consumer's ATM card without proper authority and prior approval from the supervisor.
