    We are holding tryouts for our Dragons teams at Kramer Ball Fields, Kent on Sunday, July 28, 2024. Baseball tryouts will be 1-3pm with registration starting at 12:30.     I acknowledge voluntary participation in the Dragons Youth Sports Tryout Session. I understand that while participating in this evaluation, there is a risk of injury. I understand that such an injury can range from minor to major injury, including but not limited to, paralysis, permanent bone or joint injury, scars, other chronic disabling conditions, and even death. I hereby waive any and all claims, causes of action, right to entitlements, suits or damages against the Dragons Youth Sports Organization, including any and all coaches or representatives of Dragons Youth Sports Organization as a result of, or in conjunction with my participation of this
    evaluation.

     

    I also waive any claims to any other services, uniforms, equipment, medical or training services and the like. I verify that there exist no physical disabilities, impairments, or chemical dependencies that may inhibit participation in baseball or softball sport activities. I do not know of any medical reasons tlimiting participation in this evaluation. I hereby accept and assume the risk of injury and understand the possible consequences of such injury. I, the undersigned, have read this form carefully and fully understand all items.
