    Surgical Consent

    This form serves as a signed informed consent for your pets' surgery, which we must have before performing surgery.


  • For Lumpectomy Patients:

    Please provide us with the location of your pets lumps, if you are able to mark them on your pet with a sharpie marker the morning of surgery it is very helpful for us!
  • Bloodwork and additional procedures:

  • All dogs and cats will be given fluids during surgery, as our doctors believe this will ensure the safest anesthesia. Intravenous fluids are done in nearly all human surgery, and can help to regulate blood pressure, protect organ blood flow, speed up recovery and help pets feel better after their procedure. They also provide us with access to a vein in the unlikely event of an emergency during anesthesia. The doctor may opt for subcutaneous fluids for short procedures in cats rather than IV fluids.

    We recommend bloodwork before all anesthesia, to screen for kidney and liver function, among other things. Blood and urine can be checked prior to surgery and this will allow us to tailor the anesthesia protocol if needed.

  • Your pets discharge appointment:

    Once your pet is finished with surgery we will send a text message to the number listed above, letting you know how the surgery went! We require a discharge appointment so that a technician is available to go over your pets discharge instructions after surgery, this is also your time to go over any questions. These are usually set based on your pets recovery, so the time selected will *NOT* be confirmed until you receive a text message from a technician *AFTER* your pets surgery. We will do our best to accommodate the time selected. Please select a time below that may be best for you to pick up your pet, please do not select "anytime is fine" unless you are free to pick your pet up anytime. Our latest pick up time is 4:30pm, so please be prepared to pick your pet up prior to this time. Please ensure that you have no other commitments the afternoon of surgery, as we cannot assure your pet will be ready early, as we cannot rush the recovery process.
  • Informed Consent

    I understand that some risks always exist with anesthesia and/ or surgery and that I am encouraged to discuss any concerns I have about those risks with the attending veterinarian before the procedure(s) is/are initiated. This risk may include serious bodily injury or death. I understand that it may be necessary to provide medical and/or surgical procedures which are not anticipated for the safety or care of my pet. I hereby consent to and authorize the performance of such altered and/or additional procedures as are necessary in the veterinarian’s professional judgment. I accept responsibility for any result in additional charges.
    I understand that the attending veterinarian will make every effort to contact me regarding treatment in the case of unforeseen emergencies.

    My signature below indicates I authorize and request the surgical procedures and care as indicated above, and understand I am responsible for payment for all services upon discharge of my pet from the hospital.   

  • Please note that all spays are subject to a potential "in heat/pregnant/mature or complicated fee" this can occur due to your pet being in heat close to the time of surgery, pregnancy, or things that may not be evident until the time of surgery. If your pet is a female canine over the age of two it is likely that this fee will be applied to your invoice, although we will not be able to determine this until the surgery is taking place.

    We will send your pet home with discharge instructions for post-surgery care.  We will also send your pet with any medications that are indicated for your pet.  Please ensure all medications are given as labelled.  

    If you have ANY concerns with your pet after surgery, it is important that you address these.  If these occur after hours, there are emergency clinics in Calgary who offer 24 hour care, and we encourage you to have your pet seen if you have concerns about their health outside of clinic hours.
