Participant Consent & Media ReleaseI voluntarily agree to participate in the Voices of Frenchtown Intergenerational Historical Bench Project conducted by Divine Revelations Ministries, Inc. (DRMI). I understand that my participation may include audio recordings, video recordings, photographs, and written materials documenting my memories, experiences, and reflections.
By signing below, I grant DRMI permission to:
- Record my interview.
- Edit recordings for clarity, sound quality, and length while preserving the intent and integrity of my story.
- Publish approved audio recordings on the DRMI website.
- Publish approved video recordings on DRMI's YouTube channel and other educational or promotional platforms.
- Generate QR codes that provide public access to my approved interview.
- Use my name, voice, likeness, photographs, interview excerpts, and approved materials for educational, historical, community engagement, grant reporting, fundraising, and promotional purposes related to the Voices of Frenchtown Project.
- Digitize (scan or photograph) any photographs or memorabilia that I voluntarily provide, while returning original materials to me unless otherwise agreed.
I understand:
- My participation is completely voluntary.
- I will receive no financial compensation for participating.
- I may decline to answer any interview question.
- The stories I share are based on my personal memories and experiences.
- If DRMI offers participant review before publication, I will have the opportunity to review my interview prior to its public release.
- I may withdraw from the project at any time before my interview is published by notifying DRMI in writing. Once materials have been publicly released, shared, or archived, DRMI cannot guarantee the removal of copies that have already been distributed or accessed by others.
I certify that I have read this agreement, understand its contents, and voluntarily grant these permissions.
