I understand the process may include appropriate background checks, including State and County Criminal History report, Certified Nurse’s Aid Registry Search, and a Sex & Violent Offenders Directory Search. A conviction record will not necessarily disqualify me as a volunteer. If driving will be involved, a driving record and proof of insurance may be required.

I grant Noble the right to use my photograph, voice or image, with or without my name, for any and all purposes, including, but not limited to, presentations on radio, television, social media, or in other public or private places, advertising, publicity and promotion relating thereto.

The volunteer relationship can be terminated, with or without cause and without notice, at any time, at the option of either the volunteer or Noble.
