  • All Involuntary terminations must be reviewed and approved by the Human Resource Director before any action is taken. 

  • Access Termination- Terminating Access to specific information & tools for a current Noble Employee. 

    • Request is completed by a Department Director when an Employee LEAVES a department, changes roles, no longer needs access to specific network drives, group email list, etc.   
    • Termination to the following applicable tools:
      • AWS- Employee's position no longer requires access to AWS.  EX: Administrative or Management position drops down to a DSP  
      • Network Drive Access- An Employee leaves a department and no longer should have access to the department's information.  EX: An employee in Fiscal moves to Community Living.  The employee should be removed from the Fiscal Department drives.  
      • Noble Phone number- Is there a phone number assigned to the role? If the number is assigned to a role (EX: HR Recruiter, Accounts Payable, East Program Director) If the phone number is assigned to a role they are vacating- the employee should have the Ring Central account/ number removed. 
      • Acceltrax Manager Permissions/ Unlock Employee- Supervisors need Acceltrax Permissions so that they can "unlock" an employee so edits can be made outside of the 48 hour lock period.  If an employee moves from a Supervisory role to a non-supervisory role, this should be removed.  
      • BDDS Portal Access- If an employee currently has portal access but is leaving the department or role.  Then Access should be removed. 
      • Provide Report Access
      • Door Code- Do they have a door code, and will their new position require it? 
        • Will they be working out of Noble East and need access to the building outside of normal business hours? (Directors/ Community Living) 
      • Alarm Code- Are they responsible for opening or closing a building?  Will they need access to a building outside of normal business hours? 
      • Department specific requests- Specific Programs used by departments, ex: Quickbooks, eTapestry, Adobe Pro
      • Email Group- Are they part of your department email group?  If leaving your department, they should be removed.

    Access Termination

    • This form is submitted by the Program Director when an employee leaves the department and/ or a role and access to specific information & tools needs to be terminated for the Noble Employee. 
    • EX: no longer needs AWS, Specific Drive Access, Noble Phone number, Acceltrax Manager Permissions, Unlock Employee (for supervisors), BDDS Portal Access, Provide Report Access, Door Code, Alarm Code and department specific requests.
    • When submitted the form is sent to Promethius, Accounts Receivable and Client Information Manager and the Director of Business Process Improvement.


    Establish Access for a New or Current Employee

    • This form is submitted by the Program Director when an employee is hired/ transferred into the department and/ or a role and requires access to specific information & tools.  Examples of access needs: AWS, Specific Drive Access, Noble Phone number, Acceltrax Manager Permissions, Unlock Employee (for supervisors), BDDS Portal Access, Provide Report Access, Door Code, Alarm Code and department specific requests, added to a department email group.
    • When submitted the form is sent to the Vice President of Compliance and Operations. The VP will review the form and either approve or deny the request. Once approved the form is then routed to Promethius, Accounts Receivable and Client Information Manager, Director of Business Process Improvement and to the reporting Program Director. (If denied, the form is returned to the Program Director) When submitted the form is sent to Promethius, Accounts Receivable and Client Information Manager and the Director of Business Process Improvement.

     

  • Access Request- Requesting access to specific information & tools for a new or current Noble Employee.     

    Establish Access to the following applicable tools:

    • AWS- An employee needs AWS if they need Network Drive Access
    • Network Drive Access- Each program has a drive dedicated to their program.  Access is limited to each drive and must be requested using this form. 
    • Noble Phone number- Is there a phone number assigned to the role? Does this employee have an office?  Does their position require them to have a separate number?  Do they need a physical office phone, or do they just need a phone number assigned to them so they can use their Noble phone number via the Ring Central app?
    • Acceltrax Manager Permissions/ Unlock Employee- Supervisors need Acceltrax Permissions so that they can "unlock" an employee so edits can be made outside of the 48 hour lock period.   
    • BDDS Portal Access- Does the position require client file management?  Will they be responsible for uploading documents to Provide?  Will they be responsible for completing reports/ plans and uploading to the BDDS portal.  (Managers, Directors, BC's, MUTH/ RETH, Nurses) 
    • Provide Report Access
    • Door Code- Will they be working out of Noble East and need access to the building outside of normal business hours? (Directors/ Community Living) 
    • Alarm Code- Are they responsible for opening or closing a building?  Will they need access to a building outside of normal business hours? 
    • Department specific requests- Specific Programs used by departments, ex: Quickbooks, eTapestry, Adobe Pro

  • Once you collect the equipment, what will you do with it?
  • Important!  The employee must have access to the phone number listed and a computer on the date and time noted above.  (Assigned, passthrough, trainig lab)  Promethius will call the employee on the date/time noted and will help them set up their computer and access.  

    If the employee does not have access to the phone number or their assigned computer on the assigned date and time, you will need to instruct the employee to reach out to Promethius to arrange a time to meet with Promethius. 

    It is recommended that you are present and can ensure they have access to all drives, assigned programs, printers and any other item they may need to complete their job duties.
