General Program Activities Authorisation Form

The program outlines the activities and theme of what is happening during that day at Vacation Care, however we have time throughout the day to have spontaneous activities and unplanned activities including programs and workshops onsite, activities suggested by children, free play, and playing outdoors in nature. This authorisation form allows your child to participate in these activities each day during Vacation Care.

I give permission for my child to attend the days selected and participate in all activities on the program.I acknowledge that spontaneous activities may be provided and I hereby give permission for my child to join in.I acknowledge that my child can participate in activities in OOSH including: water play, mud play, gardening, swings, bike & scooter riding, cooking, zip/slack lining, tyre swings and tree climbing.Other onsite programs include Super Soaker, OOSH Christmas, Mystery Box Challenge, Splashtastic Water Fun, Challenge your Educator, Sideshow Alley, Ninja/Martial Arts, 9D Mobile Cinema, Gecko Sports Box-FIT, Immersive Nature Experience, Little Rippers Skate Coaching, Cooking, Pickleball and End of Holidays Party Day.I acknowledge children are not allowed to bring mobile phones or a smart watch (unless it is set to school mode) to Vacation Care unless stated on the program.I acknowledge that children attending Vacation Care can bring their own wheels e.g., bike and/or scooter (NO wheelie shoes or skateboards brought from home). A helmet must be supplied and worn while riding.I acknowledge that EHOOSH is not liable for damages, breakage, or loss of personal belongings, including toys, wheels, equipment, or devices.

Berry Island Cultural Tour Excursion Authorisation Form - Friday 16th January

The excursion will be held at Berry Island Reserve. Children will be away from the service from 9:00am and return by 2:40pm. The ratio for this excursion is one educator to eight children and the anticipated number for this excursion is 50 children with nine educators. Children will be supervised by EHOOSH educators at all times. A risk assessment has been completed and is available at the service upon request. If you have booked your child in for this excursion, please fill in the details below before moving on.

I give permission for my child to attend the excursion to Berry Island Reserve on Friday 16th January, 2026.I acknowledge that Glenorie coaches private bus might not be fitted with seat belts.Arrive by 9:00am, the bus will not wait for late families as we have a scheduled booking.I acknowledge that my child is required to wear appropriate clothing, including enclosed shoes and socks. Additionally, I will ensure to pack a small backpack containing a hat, towel, change of clothes, a drink bottle, and meals.I acknowledge my child will be away from the service from 9:00am and return by 2:40pm. Lunch will be consumed at the site.I acknowledge that if my child arrives at the service without an EHOOSH t-shirt and hat they will be given a new one and my account will be charged.I understand the importance of sun safety for my child and understand that they will be required to apply sunscreen and reapply it every two hours during the designated period. The excursion will be held at Bobbin Head, Kuring-gai National Park. Children will be away from the service from 8:30am and return by 3:15pm. The ratio for this excursion is one educator to eight children and the anticipated number for this excursion is 60 children with nine educators. Children will be supervised by EHOOSH educators at all times. A risk assessment has been completed and is available at the service upon request.
