  • Social Media Release

    We love to take photos during our walks/visits and share them on our social media channels. We like to share your pet's experience with us and will also communicate this on social media with your permission.By signing this release form, you are permitting us to share your pet's photos and experiences on social media and for use in our marketing and future promotions.We completely understand and respect your decision if you would prefer us not to use photos of your dog online.

  • Veterinary release

    Please note this form will also be given in paper form and will require a wet signature.
  • I agree to the following statements in case of an emergency at Ellie‘s Happy Paw Services. In the event my pet needs emergency veterinary care due to an accident or illness, I permit Ellie‘s Happy Paw Services to seek emergency veterinary care for my pet/s if every effort has been made to contact me and I am not present. I understand Ellie‘s Happy Paw Services works hard to prevent accidents and injuries and understand that due to the nature of such animals, injuries and illness may occur at any given time. I understand that efforts will be made to seek veterinary care from my listed vet on my introduction forms, but if this is not possible, I give permission for an alternative veterinary practice to treat my pet. I also give permission for the alternative vet to have access to my pet's medical records. I give my permission for my pet to be safely transported to the veterinary clinic or for on-site care to be provided. I will assume full responsibility for the payment and of any veterinary care rendered. I agree to allow Ellie’s Happy Paw Services to make informed decisions on treatment of my pet/s if I am not reachable. With the understanding that decisions will be made in the best interest of the animal. I understand Ellie‘s Happy Paw Services will not be held liable for any injury or loss and is released from any responsibility related to transportation, treatments given, or any costs that may arise, unless negligence of the dog walker can be proven.

    Please read carefully to the below T&Cs.  
  • 1. SERVICES PROVIDEDThe Dog Wedding Chaperone agrees to supervise, handle, and care for the Client’s dog(s) during the agreed‑upon event, including assistance with ceremony participation, photos, crowd management, and basic care (water, potty breaks, calming support).2. CLIENT RESPONSIBILITIES Client confirms that:The dog is healthy, well mannered, and suitable for a public eventAll disclosed behaviour information is accurateThe dog is legally permitted at the venue3. SAFETY & BEHAVIOUR CLAUSE The Chaperone reserves the right to remove the dog from any situation that is deemed unsafe or overly stressful for the dog, guests, or staff. This includes excessive noise, crowding, or unexpected aggression.4. LIABILITY WAIVER Client assumes full responsibility for any injury, damage, or loss caused by their dog during the event. Client agrees to release and hold harmless the Chaperone from all claims, except in cases of proven gross negligence.5. EMERGENCY CARE In the event of a medical emergency, Client authorises the Chaperone to seek veterinary care. All associated costs are the responsibility of the Client.6. PAYMENT & CANCELLATION Owner agrees to a 50% non-refundable deposit to secure dates and understands the remainder of the balance should be paid no less that 4 weeks in advance of the wedding date. Cancelations with less than 4 weeks notice will result in full amount payable.
