  • 2025-2026 Enrollment Agreement

  • Tuition Obligation: TUITION IS BILLED MONTHLY AND IS DUE AND PAYABLE BY THE 1ST OF EACH MONTH.  There are two options for billing:  

    EQUALIZER PLAN - The amount per month will be the same regardless of the number of weeks in school each month.  The amount is based on the weekly tuition amount multiplied by the number of weeks in school (35) divided by the number of months (10.)

    WEEKS PER MONTH - The amount per month will be based on the based upon the number of school weeks in each month.  

    If my child will not be in attendance for a month or more due to illness, travel, vacation, suspension or other situation I understand that I am still responsible for the full payment of tuition while missing school in order to hold their spot in the class until they return.  If tuition is not paid, the spot will be filled by a child on the waiting list.  Should the school need to close for a week or more due to epidemic/pandemic exposure, acts of God, disaster, etc, there will be no refunds.

    Discounts: 
    ACTIVE Duty Military - 10% tuition discount on one child
    Multiple Children from same household - First child-Full Tuition and 2nd child-10% off tuition

  • I understand that if I have signed up to bring cash, check or Zelle and have not made payment by the first Friday of each month, I am subject to a $25 per week late fee.

  • Sick Policy: If my child is sick or not feeling well, I will keep them home FOR AT LEAST 24 HOURS after symptoms have disappeared, WITHOUT the use of medication. If I have to give my child medicine to feel better, I will not bring them to school but will keep them home until there are no more symptoms and my child feels better without the use of medication.  If my child becomes sick at school and has to go home, I will have them picked up immediately and will keep them home for at least 24 hours after symptoms have disappeared.  Click here to read complete ILLNESS GUIDELINES.

    Biting/Spitting/Disrespecting Adults:  I confirm my child does not bite or spit on siblings or other children and is respectful of adults. If my child exhibits these behaviors at school, I will be requested to pick up my child from school immediately and they may be withdrawn from the school after the FIRST incident.

    Potty Time: I confirm that my child is 100% potty trained and has mastered using the bathroom and wiping themselves without any help or supervision.  If they have an accident, I will be called to come clean up my child. Should my child have 3 accidents within 30 days, their enrollment may be suspended or withdrawn.     Behavior Policy: I confirm that my child does not throw tantrums, harm siblings or peers, and is respectful to adults. If my child exhibits these behaviors at school, is continually disruptive in class, or requires constant one-on-one monitoring by the teachers, they will have a Behavior Incident Report drawn up for my signature.  My child may be withdrawn from the school at the teacher or director's discretion for repeated or extreme reports.  Click here to read complete BEHAVIOR AND GUIDEANCE POLICY.

    Lunch: I will provide a nutritious lunch from home with no sweets or juice each day for my child. No deliveries from Door Dash, Uber Eats, restaurant delivery or other.  I will provide a snack for the class once a month, based upon the Snack Calendar provided by my child's teacher.

    Items Brought from Home: I hereby release, indemnify, and hold harmless Estrella Mountain Preschool and its staff from any loss or damage to toys, clothes, jewelry, or any other personal articles brought from home.

  • I have read this agreement and the guidelines listed above and in the parent handbook (click here to review) in their entirety and agree to follow the policies and procedures of Estrella Mountain Preschool.  Once signed, this agreement will constitute a binding contract.

  • EMERGENCY INFORMATION

    I authorize the following individuals to pick up my child from the facility if I am not available (Must list 2 people - NOT PARENTS/GUARDIANS.) This is requirement and you may not put N/A. Emergency contacts do not have to live within the immediate area. Enrollment will not be confirmed until complete.
  • Medical Information

    Please provide immunization and medical information
  • All students must be up to date on immunizations and provide a copy of child's current immunization record, Medical Exemption Form, or Religious Exemption Form by August 1. This can be scanned and emailed to preschool@emcaz.org.

  • COMMUNICATION

    We provide various forms of communication to keep you informed of upcoming events, updates to the calendar and emergency notifications.


  • SCHOOL COMMITMENT FEE

    A NON-REFUNDABLE school commitment fee will be charged today, manually by the director through an electronic funds transfer from the bank account below in the amount of $150 per family. If there is no correct and verified bank account information provided, I understand that my application will not be considered for enrollment.
  • In the event there is a any balance due after the last day of school, Estrella Mountain Preschool reserves the right to charge this account for the remaining amount due.

  • After you click "Submit" you will immediately receive a message that your form has been submitted.  Your child's enrollment is not confirmed until the application has been reviewed by the Director and an email confirmation has been sent.
