  • Please provide the contact information for the adult supervisor/sponsor of the organization or student body entity for which you hold your qualifying leadership position. After you submit this information, this individual will be sent a link requesting a recommendation letter from them confirming your position, leadership qualities, personal attributes, etc.


  • Teacher Contact Information

  • Please provide the contact information for a teacher you had (or have currently) during high school who you would like to write a USSYP recommendation letter on your behalf. The teacher providing the Teacher Recommendation may NOT be the same person who provides the Organization Supervisor/Sponsor Recommendation. After you submit this information, this individual will be sent a link requesting a recommendation letter from them confirming your academic achievement, leadership qualities, personal attributes, etc.

    Proctor Identification Information
  • A part of the USSYP selection process includes taking a proctored exam (about one hour) in a room alone with your school's USSYP Coordinator or your Guidance Counselor. You may not receive any content assistance when taking the test. Your regular Social Studies courses combined with your interests in government have prepared you for this exam - there is no study guide. After submitting this form, your school proctor will receive information about the exam via email. All exams must be taken and submitted by 5:00 pm on October 6th.

  • Upon submission of this first step of the USSYP Application, your parent/guardian, principal, organization supervisor, and listed teacher will be sent their respective verification forms and requests for a recommendation letter on your behalf. Reminder - recommendation letters are required to come from your organization supervisor (where you hold your qualifying leadership position), and from a teacher that you designated on this form. This can not be the same person. Ex: If your ELA teacher is the supervisor of NHS and you are the NHS vice president, that can not be the teacher that you choose for your Teacher Recommendation since they are also being asked to submit a recommendation.
