Dear Parents/Guardians,
We are pleased to let you know that Evergreen will be conducting hearing and vision screenings this fall. With parental permission, students will participate in one or both of the following screenings:
- Vision Screening - intended to recognize vision concerns as early as possible
- Hearing Screening - intended to identify potential hearing problems and determine the need for further evaluation

Staff from Evergreen and MAHEC will administer these screenings.
If a concern is noted during the screening process, you will be notified with
additional information, including a referral to have your student evaluated by a
health care provider, if needed.
New this year, we need your permission for your student to receive these valuable screenings. Please sign below to indicate that your child may have their vision and hearing screened.
