  • This is a lengthy form! Before you continue, make sure that you are prepared to provide the following:

    • Which weeks {studentName} will be attending
      • We are open June 22-26, July 6-10, July 13-17, July 20-24, and July 27-31.
    • Health and allergy information for {studentName}.
    • A recent headshot
    • Contact and pick-up list information for you and at least two other people.
    • If necessary, information on and a picture of anyone that is specifically not allowed to pick {studentName} up from Evergreen's campus.
    • Your signature on Evergreen Summer Adventures' consent forms (you must be a parent or guardian in order to do this; you can preview these forms on the website; the full text will be provided on this form)

  • AUTHORIZATION FOR EMERGENCY MEDICAL TREATMENT

    Should any injury occur, I grant permission for my child to receive emergency medical treatment from an appropriate health care provider to be selected by the adult supervisor of the activity, when, in such supervisor’s opinion, the need for such treatment is immediate, and when efforts to contact me are unsuccessful. I also agree to pay and be responsible for all medical, hospital or other expenses which Evergreen Summer Adventures, ECCS, and/or adult supervisors may incur as a result of securing such treatment. This remains in effect until the child is withdrawn from Evergreen Summer Adventures.

     

    HOLD HARMLESS AGREEMENT

    I agree and hereby do release and hold harmless Evergreen Community Charter School, Evergreen Summer Adventures, and/or any all adult supervisors for the activity from and for all liability which may arise from damages, loss or injuries, either to person or property, which my child may sustain while engaged in activities conducted on or off campus including, but not limited to, any damages, loss or injuries that may be sustained through transportation to or from the activity.

     

    CONFIDENTIALITY STATEMENT

    If volunteering at Evergreen Summer Adventures or Evergreen Community Charter School, you will at times be exposed to confidential information regarding students and/or staff. This information can come to you in a variety of forms, such as verbal or written information or an incident that you observe. It is imperative that such information be kept confidential for all parties involved. By signing below, you agree that you will keep confidential any such information you witness while volunteering at Evergreen Summer Adventures or ECCS.

     

    PARENTAL PERMISSION FOR RELEASE OF STUDENT LIKENESS OR WORK PRODUCTS

    I hereby authorize and grant staff of Evergreen Summer Adventures the right to use my child’s name, image, and/or work to be used in school publications and the school web site and other school-affiliated social networking sites. I also agree to allow my child to be interviewed and/or photographed by representatives of external news media in relation to any coverage of Evergreen Summer Adventures activities. I understand that by signing this I waive all compensation rights to the use of the above stated materials. This release is effective as long as my child is enrolled in Evergreen Summer Adventures.

  • These conditions are requirements of Evergreen Summer Adventures participation. You will not be able to continue with the form without consenting. If you would like to speak about it before consenting, please contact the program directors.

  • ADVENTURE WAIVER

    Acknowledgment of and Assumption of Risk and Release and Indemnity

    This release form has legal consequences. I read it carefully before I signed it.

    I certify that my child, {studentName}, is in good enough physical condition to use the Beanstalk Forest, the Climbing Wall, and/or bicycles (either their own, or borrowed from Evergreen or from other campers).  I may cease my child’s participation in this activity or decline to their participation at any time.  I am aware that ropes courses, rock climbing, and training in an indoor gym and bicycling are dangerous activities, which carry a significant risk of serious personal injury or loss of life.  

    Some of these risks include but are not limited to, bad decision making, actions of other climbers, actions of other riders, failure of equipment, rope burns or abrasions, breaking a bone, falls from or contact with walls including on artificial surfaces, climbing holds which may have become loose or damaged, falls from or contact with the ground, obstacles or other riders or accidents which cannot be foreseen. I also know that there are natural and environmental conditions, which independently or in combination with my activities, may cause severe or even fatal injuries to me or to others.

    I understand Evergreen Community Charter School has agreed to provide reasonable supervision of participants and belayers and adhere to the safety rules which are standard to the industry of ropes course and climbing wall facilities.  The guides have been trained and are committed to complying with the rules and training that has been provided.  No one is allowed to guide or belay without having undergone the training provided by Evergreen Community Charter School.

    My child agrees to pay attention to the staff, including volunteers, and to the conditions of the course, the anchors and the climbing and/or bicycles, helmets, and biking equipment that my child may be used as well as the condition of the cables. My child will report to a staff member or volunteer any damage or concerns that they notice or see. My child agrees that they will follow any rules that are posted and any other additional rules presented by the staff or volunteer. If a staff member or volunteer makes a specific request, my child agrees to promptly comply with the request.

    I want my child to participate in these activities with knowledge of the dangers involved and hereby agree to accept any and all risks of injury or loss of life. I agree to accept all responsibility for the risks, conditions, and hazards, which may occur whether they are known or unknown.

    In consideration of the opportunity to participate, I hereby waive, release, discharge, hold harmless, promise not to sue, and shall defend and indemnify Evergreen Community Charter School, its officers, directors, agents, and employees, from and against any and all liability for damage or injury (including loss of life) that may arise from or in connection with my child’s use of the Beanstalk Forest, the Climbing Wall, and/or the bicycles provided by Evergreen Community Charter School, regardless whether such injury or damage was foreseeable, which my child has or which may hereafter accrue to my child, their heirs, legal representatives, or assigns, and from any and all damage or injury (including loss of life) that may be sustained by my child directly or indirectly in connection with my child’s participation on the Beanstalk Forest, the Climbing Wall, and/or the bicycles provided by Evergreen Community Charter School.  This release specifically does not apply to any actions by the released parties which are grossly negligent, intentional or reckless.

    I further acknowledge and agree that this release and indemnity shall be construed to be as broadly and inclusively as permitted by law and that, if any provision or portion of this release and indemnity is held to be invalid by a court of competent jurisdiction, the undersigned agrees that the remaining provisions of this release and indemnity shall remain in full force and effect.
