Authorization to Disclose Federal and State Tax Return Information
(IRC §7216 Required Disclosure)
Federal law requires this consent form before we can disclose your tax return information to a third party.
By checking “I Agree” and signing below, you authorize Express Tax Professionals to disclose your tax return information to your designated representative for the limited purposes described below.
INFORMATION THAT MAY BE DISCLOSED
Your tax return information may include, but is not limited to:
- Information provided by you for the preparation of your federal and state income tax returns
- Information contained on your filed or draft tax returns (including Forms W-2, 1099, Schedule C, Schedule E, and related schedules)
- Adjustments to income, deductions, and tax credits
- Refund amounts or balances due
- Estimated tax payments
- Filing status
- Address and contact information
- Direct deposit or electronic funds withdrawal information solely for tax filing purposes
PURPOSE OF DISCLOSURE
The purpose of this disclosure is to allow your designated representative to assist with the preparation, review, filing, and discussion of your income tax return.
LIMITATIONS
Your representative may NOT:
- Sign or file a tax return on your behalf unless separately authorized under IRS Form 2848
- Make changes to your tax return without your approval
- Represent you before the IRS or any state taxing authority unless separately authorized
- Access your tax information after this authorization is revoked
DURATION OF AUTHORIZATION
This authorization will remain in effect until the earliest of:
(1) Completion of the tax services requested,
(2) Revocation by you in writing, or
(3) One (1) year from the date of signature below.
REVOCATION RIGHTS
You may revoke this authorization at any time by providing written notice to Express Tax Professionals.
Revocation will not affect disclosures made prior to the revocation.
IRS WARNING
Once your tax return information is disclosed to a third party, federal law may no longer protect that information from further disclosure.
CLIENT RESPONSIBILITIES
You are responsible for reviewing all tax information prepared on your behalf and for notifying us immediately of any changes or errors.
You are responsible for maintaining copies of your tax records as required by law.
