2025 Scholarship Application Coach Evaluation
You are visiting this online evaluation because a student athlete on a team/program you coach is completing the application process for a Fairmont Athletic Booster scholarship and has asked you to complete this evaluation.

The Fairmont Athletic Boosters will be giving up to ten (10) one thousand dollar ($1,000) scholarships in 2025 to senior student athletes.

Each applicant must secure an evaluation from the head coach of each sport/activity (team manager, sports medicine) in which they earned or will earn at the end of the spring season a varsity letter their senior year.

Evaluations will be reviewed by the Fairmont Athletic Boosters Scholarship Selection Committee with all identifying marks redacted.

Evaluations are due (online submission) by midnight on Monday, March 31, 2025, no exceptions.

By signing, the coach agrees that the statements in the evaluation are accurate and true to the best of his/her knowledge. The coach also agrees that he/she has coached this student athlete during (but not limited to) his/her senior year and the student athlete has earned at least one (1) varsity letter.
