Fairy Tails Rescue, Ltd. reserves the right to refuse adoption to any Client for any reason. This questionnaire becomes part of our contract. All dog/puppy applications require an application processing fee of $15 to be paid upon submission.

Please understand this $15 application fee does not guarantee you will be approved to adopt the animal in which you are applying for. The processing fee is also non-refundable if approved or denied. We have one main goal - match every animal to its perfect forever home!

The fee will be applied to the adoption fee if approved for adopting. If the pet is not a match for your family, but your application was approved, we will work on finding the best match for your family. The fee and application are good for up to 6 months once submitted.
