RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT
ACKNOWLEDGMENT OF RISKS
I understand that participation in cheerleading, dance, tumbling, conditioning, stretching, fitness activities, performances, competitions, camps, clinics, practices, special events, transportation, and related activities offered by Faith Cheer & Dance Chicago involves inherent risks including falls, sprains, fractures, head injuries, concussions, physical exertion, and other serious injuries. I voluntarily choose to participate and accept all known and unknown risks.

RELEASE AND WAIVER OF LIABILITY
I release, waive, discharge, and covenant not to sue Faith Cheer & Dance Chicago, its owners,directors, coaches, volunteers, staff, sponsors, affiliates, and representatives from any and all liability arising from participation in program activities to the fullest extent permitted by law.

ASSUMPTION OF RISK
I knowingly and freely assume all risks associated with participation and certify that I/my child is physically capable of participating unless otherwise disclosed in writing.

MEDICAL AUTHORIZATION
I authorize Faith Cheer & Dance Chicago to obtain emergency medical treatment for myself/my child if necessary and understand that I am responsible for all related medical expenses

INDEMNIFICATION
I agree to indemnify, defend, and hold harmless Faith Cheer & Dance Chicago and its representatives from any claims, damages, liabilities, costs, or expenses arising from participation
PHOTO, VIDEO & MEDIA RELEASE
I grant Faith Cheer & Dance Chicago permission to photograph, videotape, livestream, and record myself/my child for promotional, marketing, educational, fundraising, website, and social media purposes without compensation

CODE OF CONDUCT ACKNOWLEDGMENT

I understand that participants and parents are expected to conduct themselves respectfully and that disruptive behavior may result in removal from the program without refund.

GOVERNING LAW
This Agreement shall be governed by the laws of the State of Illinois.
