Scholarship Agreement & Acknowledgment
Please verify that you have read and understand the following by placing your initials beside each statement:
- I understand that a member of the Scholarship Committee will contact me by phone to review this application. Initial
- I understand that all applications are reviewed by the Scholarship Committee, and I will receive a decision via email within two (2) weeks of submitting my application. I will add winshape@familychurchpc.com to my contacts to ensure the response does not go to spam. Initial
- I understand that the full cost of WinShape Camps for Communities is $225, and recipients of a financial scholarship are not eligible to receive Kick-Off Week discounts or any other promotional discounts. Initial
- I understand that full scholarships are not typically awarded, and all scholarships are granted based on demonstrated financial need and available funds. Initial
- I understand that scholarship funds are provided through generous donations from local community partners (churches and businesses). If funds are temporarily unavailable, applicants may be placed on a waiting list until additional funds become available. Initial
- By accepting a scholarship, I agree to make every effort to ensure my child attends each day of camp. Initial
- If my child receives a scholarship, I agree to contact Family Church PC at (941) 629-0444 immediately if we decide not to use the scholarship or if my child is unable to attend camp for any reason. I understand that failure to provide notice may disqualify our family from receiving future scholarships. Initial
