FCC Area 4 Family & Community Feedback
The Family Care Council Area 4 (FCC4) invites individuals, families, caregivers, providers, and community partners to share their experiences navigating services and supports for individuals with intellectual and developmental disabilities.
This form will remain open throughout the year as an ongoing opportunity to share feedback with FCC Area 4. Responses will help us identify emerging concerns, recognize what is working, guide conversations during regular FCC4 meetings, and inform our Annual Advisory Report.
We want to hear from you about:
✅ What services, supports, or resources are working well?
✅ What barriers or challenges have you encountered?
✅ Where are the gaps in services, providers, or funding?
✅ What information or resources are difficult to find or access?
✅ What changes could improve access, outcomes, and quality of life for individuals and families?
Your lived experiences help FCC Area 4 better understand what is happening in our communities and develop meaningful recommendations for system improvements.
Your privacy matters. Responses are anonymous. Feedback may be summarized and reported in aggregate, but no personally identifiable information will be included or shared.
Whether you have a success to share, an ongoing concern, an unmet need, or an idea for improvement, we want to hear from you.
Your voice helps us identify what is working, understand where the system is falling short, and advocate for meaningful solutions.
