    Federal Way Parks & Recreation Staff will review accommodation requests and communicate final decisions to the participant and/or participants' guardian. Federal Way Parks & Recreation provides reasonable accommodations to people with disabilities. If you require an accommodation to participate in a program, please complete and submit this request form. Requests should be made at least two (2) weeks prior to the date the program begins.     Please note, the ADA does not require Federal Way Parks & Recreation to take any action that would fundamentally alter the nature of its programs or services, or impose an undue financial or administrative burden.

    Individuals receiving accommodations must still comply with program behavioral expectations with or without reasonable accommodations and not present a safety concern to themselves or others.

    • The participant's Request for Accommodation does not exempt him/her from following the Federal Way Parks Code of Conduct
    • I understand it is my responsibility to provide the Federal Way Parks & Recreation Department with the most current information on myself or my child/dependent and his/her abilities to assist in making modifications to meet his/her needs.
    • I understand it is my responsibility to let the Federal Way Parks & Recreation Department know if there are any changes to the information I have provided on myself or child/dependent as soon as a change occurs.
    • I understand that my child's/dependent's Request for Accommodation does not exempt the participant from following the City of Federal Way Parks program rules and consequences. The modifications in place may assist him/her in meeting these rules, but does not exempt him/her from following them.
