      Read carefully before proceeding to the next section. Below are instructions and reminders on how you fill out, edit, and upload documents into the form.  Be sure to carefully follow the instructions and reminders or else the system will not be able to read your application form accurately.

      • Be sure to read the information about ICSP and the application process found in the website before completing this online application form. Click here to access the ICSP website.
      • For easy access, the design of application form is optimized for mobile, meaning you can use your smart phone to fill out any part of the form.  
      • You do not need to complete the application form in one sitting. You may stop anytime by clicking the SAVE button. Your application form can be edited at a later time by using the link sent to your email. Check your email and use the edit link from there.
      • Please remember to CLICK THE SAVE BUTTON AS OFTEN AS POSSIBLE to ensure that your most recent edits are transmitted to our server as soon as available.
    • There is a section in the form where you need to upload supporting documents. Only PDF files with a maximum file size of 5 MB each are accepted.  Among the documents that you need to upload are the following:

      • Certification of GPA/GWA
      • Affidavit of Residency
      • Reentry Action Plan
      • Strictly use the provided format or template for REAP, Affidavit etc.  To download the template click here.
    • For some documents e.g., Affidavit of Residency and Certification of GPA, you need to scan your documents to convert it into soft copies for uploading.  Ensure that your scanned documents are clear and readable. 

      • There are many tools for converting images (JPEG) and Docx files. Just type "convert jpg to pdf " or " convert docx to pdf " in the google search.
      • Similarly, you can also combine or merge individual pdf files into a multi-page pdf file.  Just search "merge pdf" in google.
      • Use the prescribed naming convention when uploading your documents, e.g., Dela Cruz,Joanna-CertficateOfGPA or Dela Cruz,Joanna-AffidavitOfResidency
      • Other sample and instructions of naming convention of your uploaded pdf files is shown later on "13. Upload Documents"
      • Use Proper case in filling out any part of the form.  Proper case means you write in capital the first letter of the sentence or first letter of your name. Avoid using ALL CAPS, except when it is an acronym.  Avoid using all small letters too. An application form is a formal document, so please write in that format.
      • Make sure to complete the application form before clicking the FINALIZE AND SUBMIT button.  Review your entries to ensure accuracy.
      • The finalize and submit button is hidden at first and will only be visible if you have already uploaded the required documents and filled out the various text boxes. This means that incomplete applications will not be allowed to be submitted.
      • You will not be able to edit your application once you have finalized it.  So finalize only when you are sure you have thoroughly reviewed your entries.
      • Lastly, we are setting up a private facebook group that will serve as messageboard for questions and concerns about the application.  We urge you to join this group below as we will be posting updates about the applications in this group from time to time.
