Please have a valid form of ID (Drivers License, State ID or Passport) available to upload at the end of the form.

Insurance
All renters are required to have event rental insurance. We have provided a link to The Event Helper Insurance Co. or The Event Gaurd for your convenience. Both Insurance companies specialize in event rentals or you may choose any insurance company of your choice. If you choose a different insurance company, listed below are the requirements your insurnce must meet. If you need further assistance, Please call the office at 865-483-6761.
- One million in coverage
- ORUUC named as an additional insured.
- A waiver of subrogation.

Covid 19 Warning
The "Tennessee COVID-19 Recovery Act," prohibits claims "against any person for loss, damage, injury, or death arising from COVID-19 unless the claimant proves by clear and convincing evidence that the person proximately caused the loss, damage, injury, or death by an act or omission constituting gross negligence or willful misconduct." You are assuming this risk by entering the premises.
