Farm & Forest Day: Exploring Allah's Creation

• DATE: Saturday June 13, 2026
• TIME: 11:00am-1:00pm
• LOCATION: Fitra Nature School @ 129 Fresh Ponds Rd, East Brunswick, NJ 08816
• Please complete a separate form for each Muhsen participant. The participant must be registered with Muhsen to attend.
• Due to limited parking capacity, this event is only open to the Muhsen registrant and his/her adult caregiver(s).
• Recommended Dress Code: Rain boots and a light jacket. Please apply insect repellent and sunscreen before the program. 
• This event is dedicated specifically for children with special needs. We appreciate your understanding in helping us make this program accessible to those it is intended for.
• Please note that the majority of the program will be in a fenced area and wooded trails, however, there is a large lake on the property. If your child has a tendency to gravitate towards water, please register at your own risk (precaution measures will be taken to avoid any possible accidents, inshaAllah).

Informed Consent & Liability Waiver

Fitra Nature Education, along with its teachers/volunteers will do everything in their power to maintain safety during the events; however, as with any program, there is an inherent risk for the loss of personal property or injuries/accidents. In consideration of my registered participants' attendance at this event, I agree and acknowledge that:

- Students participating in this activity will be accompanied by a teacher and/or staff person and that every precaution shall be taken to safeguard the welfare of the students during all activities, but that Fitra Nature Education cannot be held responsible in the event of any accident, illness, or injury occurring during these events.
- I understand the nature of the activity in which my child or registered individuals will be participating, and that they are expected to abide by proper code of conduct during the events.
- Participation in the events may include both foreseen and unforeseen inherent risks to which my child or registered individuals may be exposed, including but not limited to the risks of physical injury, damage to real and/or personal property, and possible illness from and/or exposure to communicable diseases.
- I knowingly and freely assume all such risks, both known and unknown, and assume full responsibility for the registered individual's participation and/or attendance at the events.
- I agree to assume all risks and responsibility for any injuries including but not limited to any bodily injury, sickness, or disease arising out of my child’s attendance and/or participation in the events. I further agree to assume all responsibility for any loss of personal property arising out of my child’s attendance and/or participation in the events. I, for myself and/or on behalf of my child, agree to release and hold harmless Fitra Nature Education and any of its staff, volunteers, chaperones, teachers, and/or employees and representatives (“Releasees”) from any and all claims, liabilities, damages, or expenses, caused by or arising out my child’s participation and/or attendance at the events, whether arising from the negligence of Releasees or otherwise, to the fullest extent permitted by law. 
In case of a medical need/emergency and if parents cannot be contacted, the School representatives are authorized to take my child to a doctor or hospital in order to secure the necessary treatment.

Confirmation BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
