** Sweet trucks spots have been filled - Only savory food trucks being accepted **

- COI listing the following as additional insured:
  Five20 Events LLC, PO Box 1058, Florence AZ 85132
- Per the City of AJ: *All vendors must have a valid city business license and tax license prior to the event & food vendors must meet all Pinal County Health Codes as well as Superstition Fire and Medical District inspection and approval of tent structures and food vendors.*
- Pay the fee at the end of the application
I understand that a maximum of seven (7) savory trucks will be in attendance and it is First Come, First Served. The first vendors to pay the fee and turn in all applicable license requirements will be approved.

- Food vendor setup time will be from 9:30am to 11pm
- Pinal County Mobile Food permit can be found here
- Maricopa County Mobile Food Permit can be found here
- City of AJ Business License can be found here
- Please call or text Keith at (520) 840-9154 once at venue for placement

- Please be onsite no later than 11am so I can get you set. No exceptions.
- A vendor load in map will be provided to you a week prior to the event along with final event details.
- Absolutely NO refunds or exchanges, no exceptions
