Hello! Welcome to my application form. Please read this carefully before filling out the form.

THIS FORM IS SPECIFICALLY FOR BOOKING TATTOO CONVENTIONS AND GUEST SPOTS!! IF YOU USE THIS TO SUBMIT FOR A NORMAL APPOINTMENT AT MY STUDIO THE SUBMISSION WILL BE DISCARDED, NO EXCEPTIONS.

Currently I am accepting bookings for:

-Villain Arts Philadelphia Convention, January 26, 27, 28

I would love to book projects featuring creatures, especially snails, toads/frogs, cats, and snakes :) Bugs, spiders/scorpions are always a blast.

I'm also very partial to projects featuring skulls, reapers, dark/witchy vibes, elves, dragons.

Vases, flowers, thorny foliage, spiderwebs, mysticals are all very fun subjects for tattoos.

I am also very happy to work with your ideas as well!

PLEASE make sure the photos are bright and well-lit, and that you are standing in an upright and neutral position, with no bending or twisting of the area selected. I want to see the ENTIRE REGION. It may be best to have someone you trust to take the photo to avoid any contorting of the body or cut-off regions. so that I have an accurate assessment of the surface and area selected. Cancelof
