Informed Consent & Acknowledgement
I, the parent/legal guardian of the child(ren) named on this form, give permission for the child(ren) to participate in the selected activities organized by FORCE 1 AFRICA, including programs held at facilities operated by schools, community partners, or other organizations in partnership with FORCE 1 AFRICA.

I understand that participation in Force 1 programs may include, but is not limited to, sports, swimming, recreational activities, educational programs such as cooking, coding, technology, and other enrichment experiences that may involve inherent risks. I accept full responsibility for these risks and release FORCE 1 AFRICA, its coaches, staff, contractors, volunteers, partner schools, facility owners, and sponsors from any and all liability for injuries, accidents, or loss of property that may occur during travel to, participation in, or return from any FORCE 1 AFRICA program, except in cases of gross negligence or willful misconduct as defined under South African law.

Medical Release
In the event of an emergency, I authorize FORCE 1 AFRICA staff or their representatives to arrange for medical treatment for the child(ren) as deemed necessary by a licensed medical professional. I understand that every effort will be made to contact me first unless immediate treatment is required.

Photo & Media Release
I grant FORCE 1 AFRICA permission to use photographs and videos of the child(ren) taken during activities for promotional, educational, or marketing purposes. These may be used on websites, social media, or printed materials. The child(ren)’s name will not be used without additional consent in line with the Protection of Personal Information Act (POPIA).

Data Privacy
I acknowledge that personal information provided in this form will be processed in compliance with POPIA and used solely for program administration, communication, and safety purposes.
