Volunteer Liability Waiver & Media Release

(1) Waiver and Release: I, the volunteer, release and forever discharge and hold harmless SHED, its board, staff, agents, and assigns from any and all liability, claims, demands of any nature, either in law or in equity, which arise or may hereafter arise for the services I, the volunteer, provide to SHED. I understand and acknowledge that this Waiver and Release discharges SHED from any liability or claims that I may have with respect to bodily injury, illness, death, or property damage that may result from the services that I provide to SHED or occurring while I am providing volunteer services.

(2) Insurance: Further, I understand that SHED does NOT assume any responsibility for or obligation to provide me, the volunteer, with financial or other assistance, including but not limited to medical health or disability benefits or insurance of any nature in the event of such injury or medical expenses incurred by me.

(3) Photographic Release: I grant and convey to SHED all the rights, title, and interests in any and all photographs, images, video, or audio recordings of me or my likeness or voice made by SHED or its agents in connection with my providing volunteer services to SHED.

Parental Release and Waiver (Minor Volunteers)

Minors under 18 years must have a parental release & waiver signed before participation as a volunteer.

Please note: all minors must be accompanied by a parent/guardian for volunteer activities, or must follow the participating organization’s child protection/abuse prevention procedures.

(1) The above minor has my permission to participate in duties assigned as a Volunteer for SHED. I am aware that this volunteer assignment may present a risk of injury. In consideration of allowing them to participate in this project, I agree to assume all risks for injuries arising out of his participation as a volunteer. I agree that SHED and all employees, officials, agents, and representatives of the organization shall NOT be responsible or liable for any injury, damage, loss, or expense, to his or her person and/or property, incurred while working as a volunteer.

(2) Photographic Release: I grant and convey to SHED all the rights, titles, and interests in any and all photographs, images, video, or audio recordings of the participant or the participant’s likeness or voice made by SHED or its agents in connection with my providing volunteer services to SHED.
