  • Parent/Carer Medication Consent Form

    All prescribed medication must be in the original container as dispensed by the pharmacy, with the child's name, the name of the medicine, the dose and the frequency of administration, within its expiry date and have the date of dispensing included on the label.

    All over-the-counter medications must be in their original packaging.

  • Statement

    • I permit the trained staff at St Edmund's to administer the prescribed medication to my child during their time at the academy.  
    • I permit the trained staff at St Edmund's to administer the over-the-counter medication to my child during their time at the academy.  I understand that it may be necessary for this medicine to be administered during educational visits and other out-of-academy activities.
    • I will contact the academy when I send over the counter medication with my child.
    • I confirm that the dose and frequency requested are in line with the manufacturer's instructions on the medicine and there have been no adverse effects in the past.
    • I will collect any unused or out-of-date medicine and supply new stock to the academy if necessary. (All out-of-date medication, at the end of each half term, will be destroyed).
    • I will inform the academy immediately, in writing, if there is any change in dosage, frequency of the medication, if the medication is stopped or if I withdraw my consent.
    • I acknowledge it is my responsibility to ensure the medical information relating to my child is kept updated on Arbor.
