Should injury or circumstance warrant the need for emergency service, I understand that the clinic will try to contact the necessary people before treatment, but will exercise the option to proceed if no one is available for clearance.

I hereby authorize the Doctor (& assistants the doctor may designate) to administer treatment as therapeutically &/or diagnostically necessary. Only In extreme emergencies will necessary treatment be given without your prior approval. In these emergency situations we will contact you as soon as possible.

PLEASE READ THE FOLLOWING AS OUR STANDARD OF CARE HAS BEEN UPDATED.

If the doctor determines upon initial intake, physical examination or results from labwork that your bird(s) require to be monitored more closely by medical staff, we reserve the right to transfer them from regular boarding to hospitalization, which is an additional cost. You or your designated emergency contact will be notified of this change.

We can provide you with the cost differences upon request.
