Important Notes

You are to wear FTA or an MBT Reversible (as long as it is an FTA brand reversible)

If you do not own one, you can purchase for $10

10min running clock - no stopped clock (Shot clock only if available)

1x TO per team per game (not in the last 2min of play)

You must supply a scorer for games

Parental Permission For Emergency Treatment

In the event of illness or accident, I give my permission for emergency treatment by qualified medical personnel for my child, and I authorize the person in charge to take my child to:
I give consent for the facility to secure any and all necessary emergency medical care for my child.
Release of Liability

Although the safety of all sport activities is the primary concern, indoor sport activities at FTA's facilities may cause injuries and/or death. I expressly assume the risk of injury, death, and/or illness arising from any cause, and agree to waive the right to pursue any claim against the Sport Center and the persons in charge.
Commitment of Payment

As the primary contact, I understand I am to pay the invoice by the due date and late payment will result in a late fee. If I withdraw my team, I am still obliged to make payment as costs to FTA are still applied. I understand that failure to pay on time may mean my team loses their position in the compeition.
Photo and Video Consent

I give permission for all players who play for my team has consent for the use of photos and videos to be used by FTA during the event/competition and beyond. This includes adveritsing, social media and more
