P.A.T.H. Program Application
Providing Assistance Through Housing
Thank you for your interest in the Forward Christian Center P.A.T.H. (Providing Assistance Through Housing) Program.

The P.A.T.H. Program is a Christ-centered transitional housing ministry designed to provide safe, affordable temporary housing while helping individuals and families move toward long-term stability through accountability, life skills development, financial stewardship, and spiritual growth.

Housing Options
The Forward Christian Center P.A.T.H. (Providing Assistance Through Housing) Program offers two housing options designed to meet the varying needs of individuals and families seeking temporary housing assistance.

Placement into either housing option is based on availability, household size, program eligibility, and the recommendation of program leadership. Submission of an application does not guarantee placement.

Option 1: Single-Family Housing
Single-Family Housing is designed for individuals and families who qualify for independent housing through the P.A.T.H. Program.

These homes provide a private living environment and are subject to the terms and conditions of the lease agreement.

Move-In Requirements
Upon approval for a Single-Family Housing unit, residents must:

- Sign the required lease and program agreements.
- Pay the required security deposit.
- Pay the first month's rent prior to taking possession of the property.
Additional move-in requirements may apply depending on the specific property.

Option 2: Shared Living Housing
Shared Living Housing is designed for individuals who are willing to live in a structured, community-style environment while working toward independent living.

Shared Living residences include:

- Private bedroom
- Shared bathroom(s)
- Shared kitchen
- Shared living room
- Shared common areas
Residents accepted into Shared Living Housing must be willing to participate in a cooperative living environment and comply with all Community Living Standards outlined in the Resident Handbook.

Additional Requirements
Upon approval for Shared Living Housing, residents are required to:

Complete the Shared Living Housing Agreement and any additional required forms.
Pay a one-time $100 Bedding & Essentials Fee, which helps provide initial household necessities and move-in essentials.
The Bedding & Essentials Fee is due prior to move-in and is non-refundable.

Completing this application is the first step in determining eligibility for the program.

Please answer all questions honestly and completely. Incomplete applications or false information may delay the review process, result in denial of your application, or, if discovered after acceptance, may result in removal from the program.

Submission of an application does not guarantee acceptance into the P.A.T.H. Program.

Application Process
Once your completed application and all required documentation have been received, the following process will take place:

- Submission of completed application and supporting documents.
- Administrative review of the application.
- Verification of information provided.
- Background screening (when applicable).
- Personal interview with program leadership.
- Eligibility determination.
- Written notification of approval, denial, or placement on a waiting list.
- Resident orientation and move-in scheduling for approved applicants.
- Applicants will be notified once the review process has been completed.

I understand that providing false, misleading, or incomplete information may result in denial of my application or dismissal from the P.A.T.H. Program if discovered after acceptance.

I understand that payment of the $75 non-refundable application fee does not guarantee acceptance into the program.

I acknowledge that I have read and understand the application process and agree to cooperate fully throughout the eligibility review process.

Application Fee
A non-refundable application fee of $75.00 is required with each completed application.

The application fee helps offset the administrative costs associated with processing applications, including but not limited to:

- Application review
- Administrative processing
- Background screening (when applicable)
- Reference verification
- Interview scheduling
- Eligibility determination

The application fee is non-refundable, regardless of whether the applicant is approved, denied, withdraws their application, or if housing is unavailable at the time of application.

Payment of the application fee does not guarantee acceptance or placement into the P.A.T.H. Program.

Application Fee Waiver
Forward Christian Center recognizes that financial hardship may create barriers for individuals seeking assistance.

Program leadership may waive the $75 application fee on a case-by-case basis for applicants who demonstrate financial hardship or other extraordinary circumstances.

Requests for a fee waiver must be made prior to submitting the application and are subject to approval by the Program Director or designated ministry leadership. A request for waiver should be emailed to theforwardcc@yahoo.com

Approval of an application fee waiver does not guarantee acceptance into the P.A.T.H. Program.
