We try very hard to match the right pet into the right family. Before committing to adopting a pet, please be prepared to keep this pet for life. It is hard on everyone, especially your pet, if s/he must go from home to home. Please be this pet's forever family! Thank you so much for your time and interest!

By completing this application you are giving written permission for Kentucky SAVE to contact: Your landlord and request a copy of your lease (if applicable), personal references, current employer, your veterinarian for confimation you are a current client and that any current pets are up to date on vacinations, heart-worm preventatives and flea/tick prevention; and all other statements/needs listed in the application or that arise during review and discussions with you.

What made you wand to become a foster parent?*

When fostering a pet, we expect you to make a commitment to the care and possible rehabilitaion of this pet. If, however, you ever find yourself unable to foster you must contact Kentucky SAVE to return the foster pet. DO NOT attempt to find a new foster for this pet.

I agree that this animal is being fostered by myself for Kentucky SAVE and will not be sold, adopted to, or given to another party.

I agree to care for the animal in a humane manner and to follow all care instructions given to me by Kentucky SAVE. This includes supplying adequate food, water, shelter, attention, and to communicate with Kentucky SAVE about medical care.

I understand and agree that Kentucky SAVE makes no guarantees about the animal's temperament and is not responsible for future damages or injuries caused by the animal.

I understand, without liability, Kentucky SAVE, may: Investigate my home and conditions under which the animal(s) is being kept; take possession of the animal(s) if, in the opinion of Kentucky SAVE, the animal(s) is receiving inadequate care, is being imporperly housed or handled, or if any conidion of this contract has been violated.

Kentucky SAVE is responsible for veterinary care for fostered pets, any veterinary care needed for these pets must be approved through Kentucky SAVE and scheduled at an approved vet by Kentucky SAVE
