CONSENT & RELEASE WAIVER

I acknowledge that participation in activities at Four Sisters Disc Golf Park involves inherent risks including, but not limited to, uneven terrain, mud, water hazards, wildlife, farm animals, fencing, trees, thorn bushes, flying discs, and other participants that may result in injury, illness, death, or property damage.

By signing below, I certify that I have read and understood the full Consent & Release Waiver provided by Four Sisters Disc Golf Park. I voluntarily assume all risks, agree to follow all posted rules and staff instructions, authorize emergency medical treatment if required at my expense, release and indemnify Four Sisters Disc Golf Park, Four Sisters Farm, Market 817, Four Sisters Kitchen + Table, the Beekman Family, and their representatives from liability to the fullest extent permitted by Alberta law, and acknowledge that failure to follow park rules may result in removal without refund.

I will not participate while impaired by alcohol or drugs. All pets accompanying our group will remain on a leash at all times. I consent to emergency medical care if necessary. Optional: I grant permission for photographs or video of our visit to be used for promotional purposes.
