*If you do not have a current veterinarian, you will be required to find one and establish a relationship with the vet office as a new client before an adoption can be completed. You will need to provide us with the name and number for your new vet office so we can call and confirm. We will accept if you are only able to be added to a waitlist as we understand many practices are not accepting new clients.

I hereby certify that all of the information provided on this Animal Adoption Application is true and accurate to the best of my ability, and that I am at least twenty-one (21) years of age as of the submission date;

I acknowledge and understand that by submitting this application I am not guaranteed approval for adoption of the specified animal in the application nor general approval for adoption from Fur the Love of Animals (FLOA); approved adoptions are at the sole discretion of the Executive Director of FLOA or an assigned member of the team in scenarios where the Executive Director is unavailable; an in-person meet-and-greet with the animal of interest is required;

If at any time the animal needs to be surrendered, I acknowledge and agree to return the animal to FLOA to minimize the stress on the animal;

I acknowledge and understand that all personally identifiable information provided on this Animal Adoption Application will remain confidential and only reviewed by FLOA staff as determined necessary.

This information helps us to determine which animal would best suit your wishes and needs of a new pet while meeting what the animal needs to thrive in a new furever home.

Please note that submitting an application does not guarantee an approved adoption for the specified animal. We prioritize the animal’s needs above all else to ensure they thrive and that the match is a success.

Pending approval, we will keep your application on file for one year from the submission date. During that time, if a compatible animal becomes available for adoption, we will reach out to see if you would be interested. If at any time you would like us to remove you from the Approved Adopters Waitlist, please let us know via email or phone call. Similarly, if you adopt an animal from another facility within that one year, please let us know so we can update our records.

Adoption Applications are processed in the order in which they are received. You will receive an email message with the next steps within 7 days.
