WAIVER
I, {name} am desirous of attending the FUTGOF Football Academy organized by FUTGOF Football Club Limited. In exchange for being able to attend the Football Academy, unconditionally and irrevocably agree as follows:
1. ASSUMPTION OF RISK: understand that participation in or attendance at the Football Academy involves inherent risks and dangers of accidents, serious personal and bodily injury. These may result from mine or other's actions, inactions, or negligence. Further, there may be other risks not known to me or foreseeable at this time. I understand and have considered and evaluated the nature, scope. and extent of the risks involved to myself, and voluntarily and freely choose to assume these risks.
I agree. and represent that understand the nature of such activities and that am of the opinion that am in good health, and in proper physical condition to participate in such activity. further agree and warrant that if at anytime believe that my health and physical condition should change so that it would be unsafe for me to continue to participate in such activity. will immedi- ately remove myself from further participation in such activity.
2. RELEASE FROM LIABILITY: fully and forever release. and discharge FUTGOF Football Club Limited, its subsidiaries, directors, officers, employees, agents, insurers, sponsors, advertisers, owners or operators of the event, facilities, equipment, and vehicles and all others involved in the Football Academy (the "Released Parties") from any and all injuries (including death losses, damages, claims (including negligence claims), lawsuits, and any other liability of any kind, or to my property, or any other person, directly or indirectly arising out of or in connection with my attendance at the event, including transportation related to the event, even if it is due to the negligence or other fault of the Released Parties.
3. COVENANT NOT TO SUE: will not initiate any lawsuit, court action or other legal proceeding against the Released Parties, nor join or assist in any prosecution of any claim for money damages which anyone may have, on account of injuries (including death), losses, or damages sustained by myself or others in connection with my attendance at the event, and waive any right to do so.
4. INDEMNITY: I will hold harmless, indemnify, and reimburse the Released Parties from and for any sums, costs or expenses (including attorney fees) incurred by any of the Released Parties or paid by them to any person (including my insurers) in connection with any accident, injury (including death), loss, or damage sustained by me or others in connection with my attendance or participation in the Football Academy, including transportation related to the Football Academy.
5. NO INSURANCE; MEDICAL EXPENSES; MEDICAL CARE: understand that FUTGOF Football Limited (FUTGOF Football Academy) does not provide me with any insurance, life, medical or liability, for any illness, accident, injury, loss, or damage that may arise in connection with my attendance at the event. authorize and permit FUTGOF Football Academy, its agents, and event personnel to administer first aid to me, emergency transportation, and any other medical treatment performed by physi- cians, paramedics, and other medical personnel, in the event of any illness, accident or injury to myself.
6. PROMOTIONS, ADVERTISING AND PROGRAMS: For good and adequate consideration, which acknowledge have received, hereby grant, permit and release to FUTGOF Football Academy the right and authority to use, see. reproduce. and distribute. quoted material, biographical information, my photograph, likeness, recorded voice or videotaped filmed appearanc- es obtained in connection with the Football Academy for promotional and advertising purposes or programs as FUTGOF Football Academy in its sole discretion will deem appropriate.
7. ACKNOWLEDGEMENTS: On behalf of myself hereby understand, agree and acknowledge:
(A) THATI HAVE HAD SUFFICIENT OPPORTUNITY TO READ THIS ENTIRE DOCUMENT;
(B) THAT I HAVE READ AND UNDERSTAND THIS DOCUMENT, AND AGREE TO BE BOUND BY ITS TERMS;
(C) THAT I HAVE NOT RELIED ON ANY ORAL OR WRITTEN REPRESENTATIONS MADE BY FUTGOF FOOTBALL CLUB LIMITED,
OR ANY OF ITS MEMBERS, OWNERS, EMPLOYEES, OFFICERS, AGENTS OR REPRESENTATIVES;
(D) THAT I AM FULLY AWARE OF, AND HAVE BEEN ADVISED OF THE POTENTIAL RISKS CONNECTED WITH MY PARTICIPATION IN
THE FOOTBALL ACADEMY, AND;
(E) THAT I AM SIGNING THIS DOCUMENT VOLUNTARILY, AND OF MY OWN FREE WILL.
