In the event I cannot be reached, I give my permission for my child to be treated by an accredited physician in an approved emergency facility. I therefore designate the Winter Weekend Director or other adult leader of the group with the authority to act on my behalf, and to hospitalize, to secure proper treatment for, or to order emergency injection, anesthesia, or surgery for my child named on this registration form.
