The complaint form is used to notify the league of serious issues to which you feel the league should be aware of. Such issues deemed reportable are issues regarding Parent Conduct, Coach Conduct, Player Conduct or Board Member Conduct. Please be specific when reporting issues. Please document any and all information which will help the league resolve these issues, including dates, times, person/s involved, along with any other information you feel is important to state. A League Representative will get back to you within 24 hours of this form being completed.
