GBN HS||Jr Tournament - Entry Form

**NOTE: You are ONLY required to enter the PWA (Participation & Waiver Agreement) submission ID one time per tournament season for each angler and Boat Captain and at your first tournament(s) registration/payment.

Registration is REQUIRED and on-line ONLY.

Single or multiple tournament registration & payment are ALLOWED.

NO submission of entry form via any other method (mail, email, etc.).

For your first tournament(s) registration ONLY: PWA (Participation & Waiver Agreement) is REQUIRED for each angler and Boat Captain, and each PWA SUBMISSION ID is required. PLEASE ensure the PWA has been completed.

NO entry submission/payment on day of tournament.

Payment is REQUIRED with form submission with CC/Debit/Digital Wallet (Restrictions apply for Apple and Google Pay - see payments section).

NO CHECKS or other method(s) are allowed for payment.

Tournament Entry Selection: Please select the tournament(s) for entry & payment. Single or muliple tournament selections are ALLOWED.

 

PAYMENT is REQUIRED for all selections at $60 per tounament {$55 entry PLUS $5 registration fee}. CC/Debit/Digital Wallet (Apple Pay or Google Pay).

**NOTE for Apple Pay:  (1) The customer’s device is running macOS 10.14.1+ or iOS 12.1+. (2) The customer is using the Safari browser and (3) The customer has a valid card registered with Apple Pay.

**NOTE for Google Pay: (1) The customer is using Google Chrome or Safari. (2) The customer has a valid card registered with Google Pay.
