WAIVER RELEASE & TERMS OF CONDITIONS
ASSUMPTION OF RISK Liability Release
I/We, the parent(s) and/or guardian of the named player, herby give my/our consent and approval to participate in any and all GAPT BASEBALL, LLC/North Texas Scout Camp activities. I/We agree to abide by the rules of GAPT BASEBALL, LLC/North Texas Scout Camp. I/We assume all risks, including unknown risks, and hazards incidents to such participation, including transportation to and from the activities; and I/We do hereby waive, release, absolve, indemnify, and agree to hold harmless GAPT BASEBALL, LLC/North Texas Scout Camp, the chartering organization, the organizers, sponsors, colleges, universities, coaches, high school ISD’s, participants, affiliates, sponsors, distributors and agents, persons transporting the named player to and from activities; for any claim arising out of an injury to the player, whether the result of negligence or from any cause. I/We, also give my permission for mine and/or my minor child(ren) photos and/or video to appear in any publications/advertisements, newspaper articles, social media, web page, of GAPT BASEBALL, LLC/North Texas Scout Camp for promotional purposes.
In consideration of being allowed to participate in, or observe, the event or activity referenced above, I acknowledge, appreciate, and agree that:
1) The risk of injury from participating or observing the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist;
2) I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation and/or observation;
3) I willingly agree to comply with the stated and customary terms and conditions for participation and/or observation. If, however, I observe any unusual significant hazard during my presence, participation or observation of the event, I will remove myself and bring such to the attention of the nearest official immediately; and
4) I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS GAPT BASEBALL, LLC, a Texas limited liability company, and their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES“), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, OR LOSS OR DAMAGE TO PERSON OR PROPERTY, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES, OR OTHERWISE, to the fullest extent permitted by law.
GAPT BASEBALL, LLC strives to recognize outstanding performance by participants in all GAPT BASEBALL, LLC Events. This recognition is delivered through social media outlets, the GAPT BASEBALL, LLC website, and information delivered to scouts and coaches during events.
With regard to the GAPT BASEBALL, LLC statement above, I do hereby acknowledge and authorize GAPT BASEBALL, LLC, a Texas limited liability company (“GAPT BASEBALL, LLC“), and its sub-contractors, affiliates, subsidiaries, and sponsors to utilize any and all photographs, videos or other likeness of my son/daughter within the GAPT BASEBALL, LLC Event as they deem appropriate in promotional materials or social media. GAPT BASEBALL, LLC has permission to post player photos, videos and likeness to social media of players, coaches, and fans participating in any event organized or hosted by GAPT BASEBALL, LLC. Roster information provided by coaches, parents, and/or players may also be used within events, on social media, and in print material in relation to GAPT BASEBALL, LLC Events.
Insurance Acknowledgement
I/WE ALSO UNDERSTAND THAT GAPT BASEBALL, LLC CARRIES GENERAL LIABILITY INSURANCE ONLY. THE PLAYER MUST BE COVERED BY PARENT/GUARDIAN/FAMILY MEMBER’s HEALTH INSURANCE. AFTER INSURANCE HAS BEEN PROCESSED BY PARENT/GUARDIAN/FAMILY MEMBER's HEALTH INSURANCE THEN GENERAL LIABILITY WILL BE ABLE TO FILE CLAIM WITH PLAYER’S CLUB/TEAM INSURANCE FIRST BEFORE A CLAIM WILL BE FILED THROUGH TOURNAMENT/LEAGUE INSURANCE.
GAPT BASEBALL, LLC/NORTH TEXAS SCOUT CAMP Liability/Photo-Video Release
GAPT BASEBALL, LLC strives to recognize outstanding performance by participants in all GAPT BASEBALL, LLC Events. This recognition is delivered through social media outlets, the GAPT BASEBALL, LLC website, and information delivered to scouts and coaches during events.

GAPT BASEBALL, LLC has permission to post player photos, videos and likeness to social media of players, coaches, and fans participating in any event organized or hosted by GAPT BASEBALL, LLC. Roster information provided by coaches, parents, and/or players may also be used within events, on social media, and in print material in relation to GAPT BASEBALL, LLC Events.
This agreement releases GAPT BASEBALL, LLC/NORTH TEXAS SCOUT CAMP from all liability relating to injuries that may occur while participating in any activities. By signing this agreement, I agree to hold GAPT BASEBALL, LLC entirely free from liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence.
I also acknowledge the risks involved in participating in any activity at The Depot. I swear that I am participating voluntarily, and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in any activity at The Depot.
By signing below, I forfeit all right to bring a suit against GAPT BASEBALL, LLC for any reason. I will make every effort to obey safety precautions as instructed and ask for clarification when needed.
I hereby agree, on my own behalf of my heirs and assigns, to waive, release and indemnify the GAPT BASEBALL, LLC and their agents, assigns or representatives, employees from and against all liability, demands, claims, suits, losses, damages, causes of action, wrongful death, any breach of duty claim or any other claim or remedy for death or injuries to persons or property arising out of or in connection with my participation in or use of any portion of the facilities a participant or spectator. This waiver and indemnification includes all claims of negligence, unless caused by the gross negligence or willful misconduct of the released parties.
I also give my permission for mine and/or my minor child(ren) photos to appear in any publications/advertisements, newspaper articles, social media, web page, of the GAPT BASEBALL, LLC/North Texas Scout Camp for promotional purposes.
I understand and agree that this waiver and release is a material inducement for GAPT BASEBALL, LLC to allow guests to participate in activities or events sanctioned by the GAPT BASEBALL, LLC. I am the host agent for GAPT BASEBALL, LLC and have authority to enter into this release. I have carefully read this agreement the release of liability and the Photography release. I have carefully read this agreement and release of liability/photo release and understand and agree to these terms.
