I understand that Grace Bible Church's Vacation Bible School (VBS) is staffed by volunteers and is a non-profit church activity. As such, if my child has behavioral issues or medical conditions that are consistently disruptive to the other students, pose a safety risk, and/or require more one-on-one attention than our volunteers can provide, the child's parent/guardian will be contacted. This may result in the child needing to be picked up from VBS early or not being allowed to return for the remaining days of VBS.

I authorize the Grace Bible Church/Vacation Bible School (VBS) staff to seek emergency medical treatment for my child if necessary.
By signing this form, I agree to the following: I, being the legal guardian of the child registered by this form, give permission for said child to attend Vacation Bible School under the direction of Grace Bible Church in Colville Washington. The undersigned, being a parent and/or guardian of the above minor, does hereby authorize the treatment by a qualified and licensed medical doctor of the above minor in the event of a medical emergency which, in the opinion of the attending physician, may endanger the minor's life, cause disfigurement, physical impairment, or undue discomfort if delayed, while said minor is participating in Vacation Bible School, including transportation to and from. This authority is granted only after a reasonable effort has been made to contact me. I also release Grace Bible Church from any liability associated with such a medical emergency. WAIVER OF LAWSUIT/LIABILITY: I also hereby forever release and waive my right to bring suit against Grace Bible Church and its owners, officers, directors, managers, volunteers, employees, or other representatives in connection with exposure, infection, and/or spread of any communicable disease, related to utilizing Grace Bible Church's services and premises. I understand that this waiver means I give up my right to bring any claims including for personal injuries, death, disease or property losses, or any other loss, including but not limited to claims of negligence and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.
