Participant Info (ADULTS ONLY)

If you are under the age of 18, your parent or guardian must complete the following section. (*Note to all the participants: any offenses to attendees will not be tolerated and those in question will be asked to leave the area immediately.)

I know that participating in the IRON UP:  FUN DAY (FUN DAY AT THE PARK WITH COVID-19) is a potentially hazardous activity. I should enter and participate in physical activities only if I am medically able and/or supervised by someone knowledgeable of my medical condition and treatment needs. 

 

I acknowledge and assume any and all risks associated with this event including, but not limited to, adhering to allergies or adverse reactions to application of face painting on the body, obeying rules and maintaining distance of others as well as safety restrictions with constant supervision by parent or guardian while playing in the bouncy house(s), obeying rules for playground and maintaining safety restrictions with constant supervision by parent or guardian while playing on playground, obeying rules and maintaining safety restrictions with constant supervision by parent or guardian while in the cash machine, obeying rules and having regard for safety of others/self with regard to foul language use as well as threats to harm someone physically and making physical contact with someone to cause bodily harm*, using alcoholic beverages of any kind while on the premises*, using a weapon (gun, knife, etc.) to cause bodily harm to others*, damaging property on the facility grounds and inside the facility*, refusing to obey the volunteers as well as organizers’ repeated requests to obey rules while attending event*, adhering to rules and maintaining safety restrictions with constant supervision by parent or guardian while inside the gaming truck, maintaining distance from the DJ equipment and obeying safety rules for protection of equipment as well as safety for self/other participants, adhering to rules of games and obeying volunteers while participating in games as well as vendor information workshops, adhering to food and beverage distribution rules for participants as prescribed by volunteers as well as organizers, and reporting any suspicious or unlawful activity to volunteers as well as organizers that could potentially cause harm to others. 

 

Knowing and accepting these risks and in consideration of the acceptance of my entry, I hereby for myself, my heirs, representatives or anyone else claiming on my behalf, covenant not to sue, and waive, release, and discharge Genèsic Nonprofit Organization as well as Albany State University in regard with the Albany State University Health Literacy Project Funding from Health Human Services and Office of Minority Health Grant, its volunteers, sponsors, and anyone else acting for or on behalf of Iron Up:  Fun Day (Fun Day at the Park with Covid-19) from any and all claims of liability for death, personal injury, or damage of any kind arising out of my participation in this event.

 

This acknowledgement of Risk and Waiver of Liability extends to all claims of every kind whatsoever. 

 

I also consent to emergency treatment in the event of injury or illness. 

 

I grant full permission to Genèsic Nonprofit Organization, Inc. and/or any person or entity authorized by it to use my name, photograph, recordings, or any other information advertising the event (www.genesicnonprofit.org)  in the public domain in regard to the Iron Up:  Fun Day (Fun Day at the Park with Covid-19). 

 

My signature acknowledges that I have read the above waiver and I agree and accept all terms and conditions set forth herein.

REQUIRED FOR ALL PARTICIPANTS UNDER 18 YEARS OF AGE:

PARENT OR GUARDIAN’S AUTHORIZATION FOR MEDICAL CARE AND CONSENT AGREEMENT

I certify that I am the parent or legal guardian of the below-named participant in the ACTIVITY.  On behalf of myself and my spouse, partner, co-guardian or any other person who claims the participant as a dependent, I have read the above agreement, I understand the contents of this Acknowledgement of Risk and Waiver of Liability, assent to its terms and conditions, and sign this Acknowledgement of Risk and Waiver of Liability of my own free will. 

 

I acknowledge that my dependent and I have agreed to the terms and conditions of my dependent’s participation in the ACTIVITY, and I hereby give my consent to participation by my dependent in the ACTIVITY, and to receive medical treatment determined to be necessary. 

 

I further agree to hold harmless, indemnify and defend Genèsic Nonprofit Organization, Inc. as well as Albany State University from and against all claims, demands or suits that my dependent has or may have.
