Waiver and Disclaimer for CPR.1st AID.AED/OSHA/IT Training Registration

By registering for and participating in this CPR.1st AID.AED/OSHA/IT Training session, you acknowledge and agree to the following:

There are NO REFUNDS for the cost of your training.

1. Voluntary Participation
I understand that my participation in this CPR/OSHA training is entirely voluntary. I certify that I am physically and mentally capable of participating in the training activities, including demonstrations and hands-on exercises that may involve physical exertion.

2. Assumption of Risk
I acknowledge that participation in CPR and OSHA training may involve physical activity and interaction with equipment. I voluntarily assume full responsibility for any risks of injury or illness that may occur as a result of my participation, including but not limited to sprains, strains, or other physical injury.

3. Medical Disclaimer
This training is educational and is not a substitute for medical advice, diagnosis, or treatment. If I have any preexisting health conditions or concerns, I understand it is my responsibility to consult with a medical professional before participating.

4. Certification Disclaimer
I understand that successful completion of the course may lead to certification based on my performance and understanding of the material. Certification is not guaranteed and is contingent upon meeting the required standards.

5. Release of Liability
I hereby release, waive, discharge, and hold harmless the organizers, trainers, venue hosts, and all affiliated entities from any and all claims, liabilities, demands, or causes of action that may arise from my participation in this training.

6. Consent to Media
I grant permission for the use of my image or likeness in any photographs, video recordings, or other media taken during the training for educational, promotional, or commercial purposes, unless I provide written notice to opt out before the start of the session.

7. Code of Conduct
I agree to maintain professional behavior and respect toward instructors and fellow participants throughout the training. Disruptive or unsafe behavior may result in removal from the session without refund or certification.
